超声定位在肥胖患者锁骨上臂丛神经阻滞中的应用

摘　　要：目的观察超声定位应用于肥胖患者锁骨上臂丛神经阻滞效果。方法选择2013年6月至2014年6月梅州市人民医院收治的需行锁骨上臂丛神经阻滞的肥胖患者60例作为研究对象,采取随机数字表法分为两组,各30例。超声组于超声定位下行锁骨上臂丛神经阻滞,盲探组于盲探下行锁骨上臂丛神经阻滞,比较两组运动阻滞效果及不良反应发生情况。结果超声组运动阻滞效果显著优于盲探组(P<0.05)。超声组麻醉效果优良率为100.0%(30/30),盲探组为76.6%(23/30),超声组麻醉效果显著高于盲探组(P<0.05)。超声组1例(3.3%)发生不良反应,盲探组6例(20.0%)发生不良反应,超声组不良反应发生率显著低于盲探组(P<0.05)。结论超声定位应用于肥胖患者锁骨上臂丛神经阻滞效果较佳,可显著降低不良反应发生率,具有重要临床价值。Objective To observe the effect of ultrasonic location in obese patients with supraclavicular brachial plexus block.Methods A total of 60 obese patients who needed supraclavicular brachial plexus block in Meizhou City People＇ s Hospital during Jun.2013 and Jun.2014 were included in the study,they were randomly divided into two groups according to random number table method：the ultrasonic group adopted ultrasonic guided brachial plexus block,while the blind detection group took brachial plexus block with blind detection,then the effect of the two groups was compared.Results The effect of ultrasonic motor block group was better than the blind detection group（P〈0.05）.The good rate of anesthesia was 100.0%（30/30） in the ultrasonic group and was significantly better than the blind detection group＇s 76.6%（23/30）（P〈0.05）.The incidence of adverse reactions in the ultrasonic group was 1 case（3.3%）,in the blind detection group was 6 cases（20.0%）,there was statistically significant difference（P〈0.05）.Conclusion The effect of ultrasonic location in obese patients with supraclavicular brachial plexus block is better,since it has smaller incidence of adverse reactions and great clinical value.
