复方α-酮酸联合醋酸钙对维持性血液透析患者钙磷代谢的影响

摘　　要：目的观察复方α-酮酸联合醋酸钙对维持性血液透析患者钙磷代谢的影响。方法将2010年1月至2014年12月绵阳市中心医院收治的99例维持性血液透析高磷血症患者采用随机数字表法分为联合组(醋酸钙+复方α-酮酸)(31例)、醋酸钙组(醋酸钙)(38例)和对照组(低磷饮食)(30例),疗程12周。比较三组患者的钙磷代谢及营养状况。结果治疗后3个月,三组患者的血磷、血钙磷乘积、血清甲状旁腺激素(PTH)比较差异有统计学意义(P<0.05),血钙3组差异无统计学意义(P>0.05);治疗后3个月,三组白蛋白比较差异有统计学意义(P<0.05),肌酐和尿素水平三组比较差异无统计学意义(P>0.05)。结论复方α-酮酸联合醋酸钙可用于治疗慢性肾脏病血透患者高磷血症,有效降低血磷、血钙磷乘积、PTH水平。Objective To investigate the effect of α-keto acid combined calcium acetate in maintaining hemodialysis patients.Methods Ninety-nine patients with maintenance hemodialysis admitted to the Central Hospital of Mianyang from January 2010 to December 2014 were randomly divided into the combined group,calcium acetate group and the control group.The combined group received α-keto acid and calcium acetate,the calcium acetate group received calcium acetate,while the control group don＇ t received α-keto acid and calcium acetate and it just to give a low phosphorus diet.Calcium and phosphorus metabolism and nutritional status were compared between the three groups.All groups were treated for 12 weeks.Results After 3months,three groups of patients with the serum P3 ＋,Ca2 ＋× P3 ＋,and PTH levels comparative difference were statistically significant（P〈0.05）.The blood calcium levels in the combined group was higher than that in the other two groups,but there was no statistically significant difference（P〉0.05）.The albumin was statistically significant（P〈0.05）.Serum creatinine and blood urea nitrogen in the combined group were lower than those in the other two groups,but here were no statistically significant difference（P〉0.05）.Conclusion α-keto acid combined calcium acetate acid can improve calcium and phosphorus metabolism disorder and can reduce the serum P3 ＋,Ca2 ＋× P3 ＋,PTH levels.
