阿托伐他汀联合曲美他嗪对冠心病NT-proBNP、hs-CRP、Fib及心脏功能的影响

摘　　要：目的探讨阿托伐他汀联合曲美他嗪对冠心病N末端脑钠肽前体(NT-proBNP)、高敏C-反应蛋白(hs-CRRP)、血浆纤维蛋白(Fib)和左心室舒张末期内径(LVEDD)、左心室射血分数(LVEF)、左心室后壁厚度(LVPWd)的影响。方法冠心病患者156例采用随机数字表法分为观察组和对照组各78例,2组均采取冠心病常规治疗,对照组加用曲美他嗪,观察组加用阿托伐他汀联合曲美他嗪治疗,疗程均为30 d。比较2组临床疗效及NT-proBNP、hs-CRP、Fib、LVEDD、LVEF、LVPWd。结果观察组总有效率为94.87%高于对照组的79.49%(X^2=3.513,P<0.05);与治疗前比较,2组血NT-proBNP、hs-CRP和Fib含量明显降低(P<0.05),且观察组低于对照组(t=3.646、4.013、2.856、P<0.05);治疗后,2组心功能均改善(P<0.05),且观察组LVEDD、LVPWd明显低于对照组(t=2.653、1.978,P<0.05),LVEF明显高于对照组(t=2.374,P<0.05)。结论阿托伐他汀联合曲美他嗪有助于降低血NT-proBNP及Fib含量,增强心脏射血能力,提高治疗效果。Objective To investigate the effects of atorvastatin combined with trimetazidine on the levels of N-terminal pro brain natriuretic peptide（NT-proBNP）,high sensitive C-reactive protein（hs-CRP）,plasma fibrinogen（Fib） and left ventricular end diastolic diameter（LVEDD）,left ventricular ejection fraction（LVEF） and left ventricular thickness wall thickness degree（LVPWd） in coronary heart disease.Methods Using random number table method,156 patients with coronary heart disease were divided into observation group and control group with 78 cases in each group,2 groups of patients were taken routine treatment,the control group added trimetazidine,observation group were given atorvastatin combined with trimetazidine treatment,treatment lasted for 30 d.Clinical efficacy and NT-proBNP,hs-CRP,Fib,LVEDD,LVEF,LVPWd were compared between the 2 groups.Results The total effective rate of observation group was 94.87%,which was higher than that of the control group of 79.49%（χ^2=3.513,P〈0.05）;compared with before treatment,2 groups＇ NT-proBNP,hsCRP and Fib were significantly decreased（P〈0.05）,and the observation group were more obviously than that in control group（t= 3.646,t=4.013,t=2.856,P〈0.05）;after treatment,2 groups of heart function were improved（P〈0.05）,and the observation group＇s LVEDD,LVPWd was significantly lower than those of control group（t=2.653,t=1.978,P〈0.05）,LVEF was significantly higher than the control group（t =2.374,P〈0.05）.Conclusion It demonstrated that the atorvastatin combined with trimetazidine can help decrease the NT-proBNP and the content of Fib,improve the cardiac function and treatment effect.
