小儿腹腔镜疝囊高位结扎术与传统疝囊高位结扎术的临床研究

摘　　要：目的探讨小儿腹腔镜下行腹股沟斜疝疝囊高位结扎手术方法、疗效及优缺点。方法回顾性分析蚌埠医学院第一附属医院用腹腔镜手术治疗90例小儿腹股沟斜疝,男72例,女18例;年龄5个月～6岁,平均2.5岁,双侧疝22例,单侧疝68例,术中发现隐匿疝12例。30例传统腹股沟斜疝疝囊高位结扎术,男24例,女6例,均为单侧腹股沟斜疝,年龄5个月～7岁,平均2.5岁。比较小儿腹腔镜下腹股沟斜疝疝囊高位结扎术与传统腹股沟斜疝疝囊高位结扎术术中、术后及并发症情况。结果腹腔镜组手术时间平均为（7.230±3.350）min,传统组手术时间平均为（18.560±5.232）min,差异有统计学意义。腹腔镜组术中出血平均为（0.856±0.753）ml,传统组术中出血（3.225±0.742）ml,差异有统计学意义。腹腔镜组住院时间为（3.125±1.216）d,传统组住院时间为（5.512±0.825）d,差异有统计学意义。腹腔镜组术后复发与传统组术后差异无统计学意义。传统组术后并发症与腹腔镜组差异有统计学意义。结论腹腔镜下腹股沟斜疝疝囊高位结扎手术治疗小儿腹股沟斜疝切口小,损伤小,康复快,手术时间及住院时间短,安全、疗效可靠;可同时发现及处理对侧隐匿性疝,并发症少,复发率低,值得临床推广。Objective To investigate the advantages and shortcomings of inguinal hernia sac ligation operation under pediatric laparoscopic. Methods Retrospective analysis of 90 cases of indirect inguinal hernia in our hospital undergoing laparoscopic operation（ male = 72,female = 18）,aged from 5 months to 6 years,and the average age was 2. 5 years old. 22 cases were bilateral hernia,68 cases were unilateral hernia,and 12 cases of occult hernia were found in operation. 30 cases with the traditional inguinal hernia who treated with high ligation of hernial sac（ male = 24,female = 6）,and all patients were unilateral inguinal hernia,aged from 5 months to 7 years,and the average was 2. 5 years old. Then compare the differences in the perioperative,postoperative,and complications between pediatric laparoscopic inguinal hernia and traditional inguinal in hernia sac high ligation. Results The operation time of pediatric laparoscopic inguinal hernia was（ 7. 230 ± 3. 350） min,the operation time of traditional inguinal with hernia sac high ligation was（ 18. 560 ± 5. 232） min,the difference were significant; blood loss of pediatric laparoscopic inguinal hernia was（ 0. 856 ± 0. 753） ml,blood loss of traditional inguinal with hernia sac high ligation was（ 3. 225 ± 0. 742） ml,the difference were significant; hospital stay of pediatric laparoscopic inguinal hernia was（ 3. 125 ± 1. 216） day,hospital stay of traditional inguinal with hernia sac high ligation was（ 5. 512 ± 0. 825） day,the difference were significant; The complications were significantly reduced in the group of laparoscopic,the difference was significant,and no obvious difference was found in the recurrence rate between these two groups. Conclusion In the treatment of pediatric inguinal hernia,the treatment of laparoscopic inguinal hernia sac high ligation have many advantages,such as small incision,less injury,quicker recovery,shorter operation time and hospital stay,safety,fewer complications,lower recurrence,efficacy as well as reliable
