三酰甘油与高密度脂蛋白胆固醇比值与绝经后女性代谢综合征的相关性研究

摘　　要：目的探讨三酰甘油与高密度脂蛋白胆固醇比值(TG/HDL-C)与绝经后女性代谢综合征(MS)的相关性。方法选取2013年8—12月在新疆医科大学第一附属医院体检中心进行健康体检的绝经后女性(≥50岁)408例为研究对象,按照2005年国际糖尿病联盟(IDF)标准将绝经后女性分为MS组和非MS组。检测两组血脂水平,计算TG/HDL-C,数据分析采用SPSS 17.0统计软件完成。结果 MS组与非MS组TG/HDL-C〔(1.70±0.77)比(0.74±0.46)〕比较,差异有统计学意义(P<0.05)。根据TG/HDL-C水平的四分位数分组,随着TG/HDL-C水平的增高,各组间体质指数(BMI)、收缩压、空腹血糖、TG逐渐升高,高密度脂蛋白胆固醇(HDL-C)逐渐降低,差异有统计学意义(P<0.05)。人工受试者工作特征(ROC)曲线分析当TG/HDL-C切值取0.903时,对MS的诊断效率最高,曲线下面积为0.882〔P=0.000,95%CI(0.844,0.920)〕,灵敏度为87.40%、特异度为74.40%。结论 TG/HDL-C可能是绝经后女性MS良好的诊断指标。Objective To investigate the correlation between triglyceride to high- density lipoprotein cholesterol ratio（ TG/HDL- C） and metabolic syndrome（ MS） among postmenopausal women. Methods A total of 408 postmenopausal women（ ≥50 years） who received physical examination in medical examination center of the First Affiliated Hospital of Xinjiang Medical University from August 2013 to December 2013,were selected as study subjects,the subjects were divided into MS group and non- MS group based on the 2005 criteria of International Diabetes Federation（ IDF）. The blood lipid level was detected,and TG / HDL- C was calculated. Statistical analyses were performed using SPSS 17. 0 software package. Results There was significant difference in TG / HDL- C between MS group and non- MS group 〔（ 1. 70 ± 0. 77） vs（ 0. 74 ± 0. 46）,P〈 0. 05〕.The subjects were divided into four subgroups according to TG / HDL- C quartiles. From subgroup quartile 1 to subgroup quartile4,BMI,systolic blood pressure, fasting plasma glucose level, and TG level increased gradually, while HDL- C level decreased gradually,the differences were statistically significant（ P〈 0. 05）. In a receiver- operating characteristic curve（ ROC） analysis,when the cut off of TG/HDL- C was 0. 903,the diagnostic efficiency for MS achieved the highest value,the area under the curve was 0. 882 〔P = 0. 000,95% CI（ 0. 844,0. 920） 〕,the sensitivity was 87. 40%,and the specificity was74. 40%. Conclusion TG / HDL- C can be used as a good diagnostic indicator for MS among postmenopausal women.
