不同剂量阿托伐他汀治疗动脉粥样硬化临床疗效的对比研究

摘　　要：目的比较不同剂量阿托伐他汀治疗动脉粥样硬化的临床疗效。方法选择2012年12月—2013年10月在资阳市第一人民医院就诊的高脂血症伴动脉粥样硬化患者140例,按患者入院顺序分为高剂量组和低剂量组,各70例,高剂量组患者给予阿托伐他汀20 mg治疗,低剂量组患者给予阿托伐他汀10 mg治疗,疗程均为3个月。比较治疗前后两组患者颈动脉内膜中层厚度(IMT)、斑块面积及血脂水平,并观察两组患者临床疗效及不良反应发生情况。结果治疗3个月后,高剂量组患者临床疗效优于低剂量组(P<0.05)。治疗前两组患者总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平及IMT和斑块体积比较,差异无统计学意义(P>0.05);治疗后高剂量组患者TC、TG、LDL-C水平低于低剂量组,IMT和斑块体积小于低剂量组(P<0.05)。两组不良反应发生率比较(4.7%比4.5%),差异无统计学意义(P>0.05)。结论高剂量阿托伐他汀治疗动脉粥样硬化的临床疗效较低剂量阿托伐他汀好,能有效降血脂、促进颈动脉粥样硬化斑块消退。Objective To compare the clinical effect on atherosclerosis between high-and low-dosage of atorvastatin. Methods From December 2012 to October 2013,140 hyperlipidaemia patients complicated with atherosclerosis were selected in the First People＇s Hospital of Ziyang,and they were randomly divided into groups A and B,each of 70 cases. A group was given high-dosage of atorvastatin（ 20 mg）,B group was given low-dosage of atorvastatin（ 10 mg）,both groups treated for 3 months. Carotid intima media thickness（ IMT）,plaque area and blood lipid were compared between the two groups before and after treatment, clinical effect and incidence of adverse reactions were observed. Results After 3 months of treatment,the clinical effect of A group was better than that of B group（ P〈0. 05）. No statistically significant differences of TC,TG,LDL-C,HDL-C,carotid IMT or plaque area was found between the two groups before treatment（ P〈0. 05）;while TC,TG,LDL-C,carotid IMT and plaque area of A group were lower than those of B group（ P〈0. 05）. The incidence of adverse reactions of A group was 4. 5%,that of B group was 4. 7%, the difference was not significantly different（ P〈0. 05）. Conclusion High-dosage of atorvastatin（ 20 mg） has better clinical effect on atherosclerosis than low-dosage of atorvastatin（ 10 mg）,can effectively reduce blood lipid,promote the shrink of carotid atherosclerostic plaques.
