摘 要:目的分析2型糖尿病患者伴不同程度视网膜病变的血脂六项变化。方法选择2型糖尿病患者158例,根据眼底照相、眼底镜或者眼底血管造影结果分为:2型糖尿病无视网膜病变组(NDR)、2型糖尿病伴背景性视网膜病变组(BDR)以及2型糖尿病伴增殖性糖尿病视网膜病变组(PDR)。记录这些2型糖尿病患者的一般临床资料,检测FPG、Hb A1c、CHOL、TG、HDL-C、LDL-C、Apo-A1和Apo-B。计量资料的3组间比较采用单因素方差分析。计数资料采用卡方检验。结果 3组2型糖尿病人之间血脂状态代谢分析:CHOL、TG、HDL-C、LDL-C、Apo-A1在3组间差异无统计学意义(P>0.05);PDR组的Apo-B水平高于BDR组及NDR组,差异有统计学意义(P=0.025),Apo-B在NDR组和BDR组间差异无统计学意义(P>0.05)。结论 Apo-B在2型糖尿病合并增殖性视网膜病变组中显著升高,提示Apo-B在2型糖尿病视网膜病变的发展过程中起到一定的作用。Objective To explore the variation feature of the six serum lipid level in different extent diabetic retinopathy patients. Methods 158 patients with type 2 diabetic mellitus were chosen in the department of ophthalmology of Beijing Shijitan Hospital. Based on the eye-ground photography,ophthalmofundoscope and fundus angiography,these patients were divided into non-diabetic retinopathy( NDR),background diabetic retinopathy( BDR) and proliferative diabetic retinopathy( PDR).Clinical data was recorded and each patient was detected fasting blood glucose( FBG),glycated hemoglobin( Hb A1c) and the parameters of serum lipid metabolism including cholesterol( CHOL),triglyceride( TG),high density lipoprotein-cholesterol(HDL-C),low density lipoprotein(LDL-C),apolipoprotein-A1(Apo-A1) and apolipoprotein-B(Apo-B). The comparison of multi-group was assessed by using analysis of variance and least significant difference t-test and enumeration data by chi-square test. Results There was no statistical significance on the differences of CHOL,TG,HDL-C,LDL-C and Apo-A1 among the three groups. Apo-B of PDR group was higher than BDR group and NDR group,the differences were statistically significant( P = 0. 025). The differences of Apo-B between BDR group and NDR group had no statistical significance(P〉0. 05). Conclusion Apo-B may be a risk factor for the development of DR. Clinicians should pay attention to the level of serum Apo-B in type 2 diabetic patients.
