摘  要:目的探讨烧伤合并海水浸泡动物感染的细菌及对抗菌药物敏感性。方法制作兔烧伤动物感染实验模型。细菌鉴定用手工法和全自动微生物分析仪,药敏试验用K-B纸片琼脂扩散法。结果创面感染的细菌有大肠埃希菌、铜绿假单胞菌、阴沟肠杆菌、鲍曼不动杆菌、溶藻弧菌、副溶血性弧菌、金黄色葡萄球菌和表皮葡萄球菌等,血液感染的细菌有大肠埃希菌、铜绿假单胞菌、阴沟肠杆菌、副溶血性弧菌和金黄色葡萄球菌。革兰阴性菌药敏敏感率>80%的有阿米卡星、奈替米星、头孢他啶、头孢噻肟、头孢哌酮、头孢吡肟、左氧氟沙星、头孢哌酮/舒巴坦和亚胺培南,革兰阳性球菌药敏敏感率>80%的有左氧氟沙星、阿米卡星、奈替米星、替考拉宁和万古霉素。结论烧伤合并海水浸泡后可导致多种细菌感染,感染出现时间早、程度重,应尽早使用抗菌药物进行治疗。Objective To investigate the antimicrobial susceptibility of bacterial infection due to burn and seawater immersion.Methods The animal infection models with burn were established in rabbits. Bacteria were identified manually and by VITEK auto microbe system. The antimicrobial susceptibility was tested by K-B method. Results The main bacteria in the wounds of the rabbits in the experimental group included Escherichia coli,Pseudomonas aeruginosa,Enterobacter cloacae,Acinetobacter baumannii,Vibrio alginolyticus,Vibrio parahaemolyticus,Staphylococcus aureus and Staphylococcus epidermidis. Escherichia coli,Pseudomonas aeruginosa,Enterobacter cloacae,Vibrio parahaemolyticus and Staphylococcus aureus were also detected in the blood. The susceptibilities of the Gram-negative bacilli to 16 kinds of antibiotics were tests,and their susceptibility rates to amikacin,netilmicin,ceftazidime,cefotaxime,cefoperazone,cefepime,levofloxacin,cefoperazone / sulbactam and imipenem were higher than 80%. The antimicrobial susceptibility tests of the Gram-positive cocci to 11 kinds of antibiotics indicated that their susceptibility rates to levofloxacin,amikacin,netilmicin,teicoplanin and vancomycin were higher than 80%. Conclusion Severe bacterial infection may develop if the burn is immersed with seawater. Combined medication with broad-spectrum antibiotics is recommended in early stage.
