氧动力雾化吸入氨溴索联振动排痰仪治疗慢性阻塞性肺疾病急性发作期疗效观察

摘　　要：目的探讨氧动力雾化吸入氨溴索联合振动排痰仪治疗慢性阻塞性肺疾病急性加重期（AECOPD）患者的临床效果。方法将68例患者随机分为治疗组（A组）34例,对照组（B组）34例。A组为震动排痰仪加氨溴索氧动力雾化吸入,B组单纯氨溴索氧动力雾化吸入。观察两组治疗后每日排痰量、肺部啰音、动脉血气分析变化,记录血白细胞恢复到正常的时间、第7天胸部影像学改变情况。结果治疗后第1~5天A组排痰量多于B组;第6~8天A组排痰量明显少于B组,两组比较有显著性差异（P〈0.01）。A组肺部啰音、Pa O2改善情况与B组比较均有显著性差异（P〈0.05）;第7天胸部影像学改变情况及血白细胞恢复到正常的时间短于B组（分别P〈0.05及P〈0.01）。结论氧动力雾化吸入氨溴索联合振动排痰仪治疗慢性阻塞性肺疾病急性加重期效果显著,明显优于单纯使用药物治疗组。Objective To explore the clinical effects of oxygen nebulae inhalation of ambroxol combined with vibratory sputum-ejection apparatus in treating acute exacerbation of chronic obstructive pulmonary disease （AECOPD）.Methods 68 patients were divided into treatment group （groupA,34patients） and control group （ group B,34 patients） .Patients in both groups were given oxygen nebulae inhalation of ambroxol,and patients in treatment group were added vibratory sputum -ejection apparatus treatment.Afterwards, the change of daily sputum volume,lung rale, arterial blood gas analysis were observed, the time of white blood cells returned to normal and the change of chest imaging 7 days post -treatment were recorded.Results 1 ~5 days after the treatment, daily sputum volume of group A was more than that of group B.6~8 days after the treatment,daily sputum volume of group A was less than that of group B obviously, the difference between the two groups was statistically significant（P〈 0.05）.There were statistical significance in both lung rale and the artery blood oxygen pressure（ PaO2 ） .The time of white blood cells returned to normal and the change of chest imaging 7 days post-treatment of group A were shorter than group B（P〈 0.05）.Conclusions Oxygen nebulae inhalation of ambroxol combined with vibration sputum-ejection apparatus had remarkable effect in treating AECOPD.
