摘  要:目的探讨结肠癌患者二氢嘧啶脱氢酶基因(DPYD)的单核苷酸多态性(SNP)与5一氟尿嘧啶(5·FU)化疗的不良反应的相关性。方法分析2013年3月至2014年5月在中山市人民医院接受治疗的结肠癌患者的临床资料。检测患者的DPYD的SNP结果,并探讨其SNP结果与化疗出现的不良反应之间的相关性。结果本研究共纳入结肠癌患者58例,平均年龄为46.7岁,14G1A、G1156T多态性位点未检测到变异;而G2194A野生型53例(91.4%),杂合型5例(8.6%);T85C野生型49例(84.5%),杂合型7例(12.1%),突变型2例(3.4%);T464A野生型56例(96.6%),杂合型2例(3.4%);G2194多态性位点不同基因型对应的胃肠道反应和骨髓抑制发生率差异未见统计学意义(胃肠道反应Х^2=0.33,P=0.85;骨髓抑制,Х^2=2.275,P=0.32);T85C多态性位点不同基因型对应的胃肠道反应和骨髓抑制发生率差异有统计学意义(胃肠道反应Х^2=10.05,P=0.04;骨髓抑制发生率Х^2=22.01,P〈0.01);T464A多态性位点不同基因型对应的胃肠道反应和骨髓抑制发生率差异具有统计学意义(胃肠道反应:Х^2=6.55,P=0.04;骨髓抑制发生率;Х^2=10.61,P〈0.01)。结论结肠癌患者DPYD的SNP与5-FU化疗不良反应有相关性。Objective To investigate the correlation of the polymorphism of dihydropyrimidine dehydrogenase gene with the adverse reactions of 5-fluorouracil toxicity chemotherapy in colorectal cancer patients. Methods Clinical data of patients with colorectal cancer who received treatment at people' s hospital of Zhongshan from 2013 to 2014 was retrospectively analyzed. The results of SNP of DPYD of the patients were detected to investigate the correlation between the results of SNP and the adverse reaction of chemotherapy. Results A total of 58 patients with colorectal cancer were retrospective analyzed. The mean age was 46.7 years old. The variation of 14G1A, Gl156T polymorphism was not detected. There were 53 cases of wild type of G2194A(91.4% ), 5 cases of heterozygous type(8.6% ) ; 49 cases of wild type of T85C(84.5% ), 7 cases of heterozygous type( 12. 1% ) and 2 cases of mutants(3.4% ) ; 56 cases of wild type of T464A(96.6% )and 2 cases of heterozygous (3.4%). There was no statistical difference in the rate of gastrointestinal reaction and arrest of bone marrow to the different genotypes of G2194 polymorphism( Х^2 = 0. 33,P = 0. 85 and 2. 275, P = 0.32 seperately). There was significant differences in the rate of gastrointestinal reaction and arrest of bone marrow to the different genotypes of T85C poly- morphism ( Х^2 = 10.05, P = 0.04 and Х^2 = 22. 01, P 〈 0. 01 ). There was significant differences in the rate of gastrointestinal reaction and arrest of bone marrow to the different genotypes of T464A polymorphism ( Х^2 =6. 55 ,P =0. 04 and Х^2 = 10. 61 ,P 〈0. 01 ). Conclusions There is significant correlation of the SNP the of DPYD in colorectal cancer patients with the adverse reaction 6f 5-fluorouracil toxicity chemotherapy.
