老年退行性椎体滑脱伴腰椎管狭窄症的手术治疗

摘　　要：目的观察手术治疗老年退行性椎体滑脱伴腰椎管狭窄症的临床效果，为后期临床治疗提供参考。方法选取阜阳市第五人民医院2011年1月至2014年3月门诊收入的30例老年退行性椎体滑脱伴腰椎管狭窄症患者作为研究对象，根据治疗方式的不同分为研究组与对照组，每组15例。研究组行单侧或双侧椎板开窗减压术，对照组给予全椎板切除减压联合植骨融合椎弓根螺钉内固定治疗。观察两组患者经治疗后的临床效果。结果两种手术方式的优良率、并发症发生率比较差异均未见统计学意义（P〉0．05）。结论两种手术治疗老年退行性椎体滑脱伴椎管狭窄均有较好的效果，术者应经验丰富，掌握手术适应证，进一步提高患者的生活质量。Objective To observe the operation treatment of senile degenerative spondylolisthesis combined with lumbar spinal canal stenosis, and to provide reference for clinical treatment. Methods Thirty cases of senile degenerative spondylolisthesis with lumbar spinal stenosis in the Fifth People＇ s Hospital of Fuyang hospital from January 2011 to March 2014 were selected as the research objects and were divided into study group and control group, with 15 cases in each group, according to the different treatment modajities. Patients in study group underwent unilateral or bilateral fenestration decompression. Patients in control group were treated with laminectomy decompression and bone graft fusion and pedicle screw fixation. The clinical effects after treatment of the two groups were ob- served. Results The differences of excellent and good rate, complication rate of operation had no significant difference （P 〉 0. 05 ）. Conclusions The two kinds of operations all have good effect on degenerative spondylolisthesis with spinal stenosis, the operator should have experience, master the indications of operation, further improve the quality of life of the patients.
