摘 要:目的了解河南省2011-2013年细菌性痢疾的病原学特征。方法以河南省2011-2013年分离到641株志贺菌为研究对象,采用血清分型、药物敏感试验及PCR检测毒力基因方法,对其进行病原学检测,以了解志贺菌菌型分布、药物敏感性及毒力基因携带情况。结果 641株志贺菌共分为2群、11种血清型,福氏志贺菌(B群)占84.6%(542/641),宋内志贺菌(D群)占15.4%(99/641),其中以F2a血清型为主的福氏志贺菌检出率由2011年的44.3%(87/196)上升到2013年57.6%(114/198),宋内志贺菌检出率由25.4%(50/196)下降到5.1%(10/198)。进行药敏试验的325株志贺菌株对对头孢噻肟、环丙沙星、氯霉素和诺氟沙星的耐药率有逐年上升趋势,差异有统计学意义(P值均<0.05);B群与D群的耐药谱有差异。进行毒力因子检测的244株志贺菌中,福氏志贺菌对set1B的携带率高于宋内志贺菌,且近几年志贺菌对毒力基因的携带率呈逐年上升趋势。结论河南省近年来志贺菌流行菌型正在发生改变,对目前常用的抗菌药物表现较高的耐药性,分离菌株中普遍携带有毒力因子。Objective This work was to explore the etiologic characteristics of shigella bacteria in Henan, 2011-2013. Methods641 Shigella isolates collected in Henan, 2011-2013 were tested and analyzed for their serotype, drug sensitive, genes of virulence factors and the related distributions. PCR was used to detect the genes of virulence factors. Results The 641 Shigella isolates were of 11 serotypes in two groups, with Shigella flexneri(B group) accounted for 84.6%(542/641) and Shigella sonnei(D group) for 15.4℅(99/641). The detection rates of S. flexneri isolates with serotype F2 a were increased from 44.3℅(87/196) in 2011 to 57.6℅(114/198) in 2013, while that of S. sonnei were reduced from 25.4℅(50/196) in 2011 to 5.1℅(10/198) in 2013. Among the 325 isolates tested for drug sensitivity, drug resistance to cefotaxime, ciprofloxacin, chloramphenicol and norfloxacin increased year by year during 2011-2013, with significant difference between group B and group D(P〈0.05). Among 244 isolates with virulence factors detected, the rate of shigella enterotoxin 1B(set1B) was higher in S. flexneri than in S. sonnei, and presented an ascending trend. Conclusion The Serotypes of shigella bacteria changed these years in Henan Province. Drug resistance of shigella bacteria was serious and the rate of virulence gene carrier was relatively high.
