糖化白蛋白检测在治疗复发性外阴阴道假丝酵母菌病中的意义

摘　　要：目的：探讨监测糖化白蛋白在复发性外阴阴道假丝酵母菌病（RVVC）治疗中的作用。方法选择2012年6月至2014年12月深圳市人民医院龙华分院收治的RVVC患者200例为试验组，另择同期真菌学检查阴性的健康体检者200名为对照组。RVVC患者行糖化白蛋白（GA）浓度测定，对超过界值的患者在抗真菌治疗的同时行糖代谢调整治疗，包括限糖饮食、运动等的试验，比较治愈后1年内复发率。结果以GA≥17.45%作为诊断界值，同时控糖治疗与仅行抗真菌治疗在RVVC复发率上比较差异有统计学意义。结论 GA检测在RVVC临床治疗有指导价值，但以GA≥17.45%为界值诊断糖尿病的RVVC治愈后1年内复发率仍然高达10.80%，高于平均5%的复发率。RVVC中非糖尿病患者平均GA升高，进一步探寻其他可能的高危因素及找到RVVC治疗中非糖尿病患者限糖饮食GA的临界点，在指导RVVC临床治疗中将有重要价值。ObjectiveTest glycated albumin in RVVC patients, explore the role of monitoring glycated albumin in RVVC therapy.MethodsRVVC patients with GA concentration measurement, patients on over cutoff line treatment in glucose metabolism adjusted antifungal therapy while limiting sugar tests include diet, exercise, etc, change the recurrence rate of cure after comparing one year.ResultsGA≥ 17.45% as a diagnostic cutoff value while controlling sugar-line treatment with antifungal therapy only difference in the relapse rate was statistically significant RVVC.ConclusionGA detection in clinical treatment guidance RVVC value to GA ≥ 17.45% recurrence rate within 1 year after the diagnosis of diabetes RVVC cure cutoff still up 10.80%, the recurrence rate is higher than the average 5%.Average GA increased in non-diabetic RVVC patients, exploring other possible risk factors and find GA critical point in non-diabetes patients will have significant value in clinical treatment with RVVC .
