胃癌变过程中凋亡基因生存素及Bcl-2的表达与幽门螺杆菌感染相关性研究

摘　　要：目的研究胃癌变过程中凋亡基因生存素(Survivin)和Bcl-2基因表达与幽门螺杆菌(Hp)感染的相关性,为临床治疗提供参考依据。方法选取医院2008年1月-2009年12月140例胃病患者送检胃黏膜组织标本,检测凋亡基因Survivin、Bcl-2的表达以及Hp的感染情况,并就其相关性进行分析,采用SPSS 19.0软件进行统计分析。结果 Hp阳性率在不同疾病胃黏膜组织中存在差异,在病变恶性程度最高的胃癌中,Hp的阳性率最高,为69.70%;胃黏膜肠上皮生化生、胃黏膜不典型增生及胃癌与慢性浅表性胃炎Hp的阳性率差异有统计学意义(P<0.05),胃癌患者Survivin、Bcl-2阳性表达率均较高,分别为63.64%、60.61%,随胃癌变发展过程呈逐渐增加趋势;相关性分析表明,Hp感染、Survivin、Bcl-2表达与胃癌变恶化程度差异有统计学意义(P<0.05);细胞凋亡指数(AI)在胃癌变发展过程中先升高后下降。结论凋亡基因Bcl-2和Survivin在胃癌变过程中有表达逐渐升高趋势,Hp感染能够诱导Survivin、Bcl-2基因表达,导致细胞凋亡受抑制。OBJECTIVE To study the expression of survivin and Bcl-2during the course of gastric canceration and explore the relationship between survivin,Bcl-2and Hp infection,so as to provide clinical references.METHODS The gastric mucosa samples of 140 patients who were admitted from Jan.2008 to Dec.2009 with stomach illness were examined.The expression of survivin and Bcl-2and the rate of HP infections were detected.Relevance analysis were conducted by SPSS 19.0software.RESULTS The Hp positive rates differed in distinct gastric illness.The Hp positive rate was the highest in stomach cancer with highest grade malignancy,with the rate of 69.70%.The differences between Hp positive rates of intestinal metaplasia,gastric mucosal dysplasia,stomach cancer and chronic superficial gastritis were significant（P〈0.05）.The survivin and Bcl-2positive expression rates were high in patients with gastric cancer,with the rate of 63.64%and 60.61%respectively.Relevance analysis showed that the HP infection and the expression of survivin,Bcl-2had a significantly positive correlation with the degree of canceration.The difference was significant（P〈0.05）.And AI showed a downward trend after the first rise in the developing course of stomach canceration.CONCLUSIONThe expression of survivin and Bcl-2increased with the developing process of gastric cancer.The HP infection will induce the expression of survivin and Bcl-2that can control the apoptosis.
