摘　　要：目的探讨早期皮瓣修复（Early flap,EF）对鼻整形（Rhinoplasty,RP）术后感染致鼻组织缺损（Nasal tissue defects,NTD）的临床效果,为临床治疗提供参考依据。方法选取2011年5月-2014年7月医院治疗的51例鼻整形患者,其中14例术后感染患者行早期皮瓣修复,观察修复后早期出现的鼻部结构改变及手术效果维持情况,用SPSS 17.0软件进行统计分析。结果共14例患者发生感染,感染率为27.45%;14例皮瓣均Ⅰ期愈合,患者均获得随访,时间4~27个月,鼻外观恢复良好,修复组织与周围皮肤在色泽、质地、光化性损害程度等方面匹配良好;2例鼻尖出现结构不对称,行皮下注射填充后解剖结构恢复。结论依照鼻整形手术亚单位原则,术后早期根据损伤性质和大小,选择合适的皮瓣修复方式,可对损伤做到良好的修复,预后较佳,治疗效果满意,恢复患者社会生活交流能力。OBJECTIVE To explore Clinical effect of early local flap on the repair of nasal tissue defect due to severe infections after rhinoplasty,so as to provide clinical references.METHODS Totally 51 cases of patients who were performed with rhinoplasty in the hospital from May 2011 to Jul.2014 were selected.Among them,14 cases who were infected after the surgery were treated with early skin flap repair.The changes of nasal structure and maintenance of the operation in the early period after the repair operation were observed.All data were statistically analyzed by SPSS 17.0software.RESULTS A total of 14 cases were infected,with the infection rate of 27.45%.And they were all cured in the stageⅠ.All the patients were followed for 4months to 27 months.The appearances of their noses recovered well and the tissue repaired were in good match with the surrounding skin in color,texture and actinicity damage degree.There were 2cases appeared asymmetric nasal tips and they were treated with subcutaneous injection and structure anatomy.CONCLUSIONRepair of nasal tissue defect should be based on the sigmasubunit principal of nasal plastic surgery,and proper flap repair according to early nature and size of the damage site can better the effect of the repair and improve the prognosis so that patients can regain their self-confidence to integrate into society.
