肾病综合征与急性肾小球炎患儿体液免疫指标对比

摘　　要：目的：对比球菌感染后急性肾小球肾炎与肾病综合征（NS）患儿体液免疫指标，以了解肾病的发病机理及指导临床治疗。方法：将48例肾病综合征患儿及32例急性肾小球肾炎患儿纳入试验组A及试验组B，并选取同期26例健康儿童，分别采用散射比浊法检测血清IgG、IgA、IgM、C3及EIISA法检测血清IgE水平。结果：试验组A与对照组比较，IgG、IgM、IgE水平均有显著性差异（P〈0.05）；试验组B与对照组比较，IgG、IgA、IgE、C3水平均有显著性差异（P〈0.05）；试验组B与试验组A比较，IgG、IgA、C3水平均有显著性差异（P〈0.05），前者IgG显著降低，后者IgG、IgA显著升高、C3明显降低。结论：急性肾小球肾炎与NS患儿临床表现不典型者可通过检测血IgG、IgA、C3水平加以鉴别，可达到早期诊断、早期治疗的目的。Objective：To compare the humoral immune indices in children with acute glomerular nephritis and nephrotic syndrome （NS） after coccus infection, to know about the pathogenesis of nephropathy and guide clinical treatment. Methods：48 cases of NS and 32 cases of acute glomerular ne-phritis children were brought into experimental group A and experimental group B, and chose 26 cases of healthy children at the same term, respec-tively used scattering turbidimetry to test the levels of serum IgG, IgA, IgM, C3 and ELISA for serum IgE. Results：Compared experimental group A with control group, all of the levels of IgG, IgM, IgE had significant differences （P〈0.05）;compared experimental group B with control group, all of the levels of IgG, IgA, IgE, C3 had significant differences （P〈0.05）;compared experimental group A with B, the levels of IgG, IgA, C3 had significant differences （P〈0.05）, the former＆#39;s IgG had significantly reduced, while the latter＆#39;s IgG, IgA had significantly increased, C3 significantly reduced. Con-clusion：Acute glomerular nephritis and NS children that with atypical manifestations can be identified by the detection of serum IgG, IgA, C3 level, to achieve the purpose of early diagnosis and treatment.
