茵陈术附汤加味治疗慢性肝衰竭阴黄证的临床研究

摘　　要：目的:观察茵陈术附汤加味对慢性肝衰竭阴黄证患者的短期干预作用。方法:将符合纳入标准的300例慢性肝衰竭阴黄证患者,随机分为试验组和对照组(各150例),对照组患者给予西医综合治疗,试验组患者在西医综合治疗的基础上加服茵陈术附汤加味,疗程均为8周。入组前比较组间基线特征,治疗8周后比较两组患者疗效指标差异及短期(3个月)的预后差异。结果:经治疗后,试验组患者在降低中医临床症状体征分级评分、恢复肝功能和凝血功能方面均优于对照组(P<0.01),且总有效率试验组为89.33%,对照组为62.67%,差异具有统计学意义(P<0.01)。结论:茵陈术附汤加味治疗慢性肝衰竭阴黄证能显著保护患者肝脏功能、改善临床症状,降低其死亡率,优于单纯西医综合治疗方案。Objective：To observe the short-term efficacy of modified Yinchenzhufu decoction in patients with chronic liver failure （CLF）.Methods：Three hundred eligible patients with CLF characterized by Yin jaundice syndrome were randomized equally to receive either western comprehensive treatment alone （control group, n =150） or in combination with modified Yinchenzhufu decoction （trial group, n =150） for 8 weeks.Baseline characteristics were compared between both groups prior to treatment initiation.At 8 weeks after treatment, pa-tients in both groups were compared to see if there was any difference in the efficacy parameters.In addition, a follow up was performed at 3 months for any difference in short-term outcomes between both groups.Results：The addition of modified Yinchenzhufu decoction was associat-ed with significantly reduced clinical symptom scores, as well as significantly improved liver function and coagulation function （all P 〈0.01 ）, compared with western comprehensive treatment alone.Overall, significantly more patients responded to the combination therapy than to the monotherapy （P 〈0.01）.Conclusion：Modified Yinchenzhufu decoction appears to have a significantly higher efficacy in protecting the liver, improving clinical symptoms, and reducing mortality in patients with CLF, compared with western comprehensive treatment alone.
