摘  要:目的通过对高血压急症患者血浆神经内分泌免疫相关因子血管紧张素Ⅱ(AngⅡ)、免疫细胞因子白细胞介素6(IL-6),免疫球蛋白G(IgG)和补体3(C3)浓度的测定,探讨高血压急症患者神经内分泌-免疫环境改变与心脑血管事件的相关性。方法选取泸州医学院附属医院2012年1月-2013年12月30例门诊健康体检者为对照组A,31例原发性高血压(非急症)患者作为对照组B,46例高血压急症患者为研究组,检测血清AngⅡ、IL-6、IgG和C3浓度。结果①研究组与对照组性别、年龄、空腹血糖及心率比较,差异无统计学意义(P〉0.05);对照组B较对照组A、研究组较对照组A收缩压均有显著升高(P〈0.01);研究组与对照组A、B舒张压比较差异均有统计学意义(P〈0.05),对照组A和B间舒张压比较差异无统计学意义(P〉0.05)。②研究组AngⅡ、IL-6、IgG及C3均与收缩压呈正相关(P〉0.05)。③研究组脑血管事件患者收缩压、AngⅡ、IgG和C3水平均高于心血管事件者(P〈0.05),而舒张压及血浆IL-6水平在脑血管事件和心血管事件患者中的发生无统计学意义(P〉0.05)。结论高血压急症患者脑血管事件发生与神经内分泌系统及收缩压水平关系较心血管事件组更为紧密,而与免疫系统无明确相关性。Objective To investigate the correlation between neuroendocrine-immune enviromental changes and organ damage in hypentensive emergency patients by detecting plasma angiotensin Ⅱ(Ang Ⅱ), interleukin-6(IL-6), immunoglobulin G(IgG) and complement 3(C3). Methods 30 healthy person from January 2012 to December 2013 in the Affiliated Hospital of Luzhou Medical College were chosen as the control group A, 31 essential hypertensive patients for the control group B and 46 hypertensive emergency patients for study group. The plasma Ang Ⅱ, IL-6, IgG and C3 were detected in all groups. Results ①Sex, age, fasting plasma glucose and heart rate had no significant differences in the groups(P〉0.05). Systolic blood presure in the control group B and the study group were higher than those in the control group A(P〈0.01). Diastolic blood presure in study group was higher than that in the controls(P〈0.05) and that in group A and group B had no significant difference(P〉0.05).②Systolic pressure in the study group was positively correlated with Ang Ⅱ, IL-6 and C3(0 〈r 〈1, P〈0.05) and diastolic pressure was positively correlated with C3(0 〈r〈 1, P〈0.05). ③Systolic pressure, Ang Ⅱ, IgG and C3 in patients with stroke were higher than in those with cardiovascular events(P〈0.05); diatolic pressure and plasma IL-6 had no statistically significance in them(P〉0.05).Conclusion Hypertensive emergency stroke has a closer correlation with systolic pressure and neuroendocrine system than cardiovascular. But it has no clear correlation with the immune system.
