摘 要:目的探讨丙泊酚复合盐酸瑞芬太尼靶控输注在介入神经放射学治疗中的效果,从而为疾病的有效治疗提供科学的理论依据。方法随机选取2013年10月至2014年10月东营市人民医院神经介入科诊治患者100例进行研究,按照随机性原则将所选患者均分为对照组和试验组2组。2组患者均给予丙泊酚复合盐酸瑞芬太尼治疗,其中对照组中的介入神经放射学治疗患者选择复合静脉泵注方式,试验组中介入神经放射学治疗患者选择靶控输注方式,其中丙泊酚的靶浓度为2~4mg/L,盐酸瑞芬太尼的靶浓度为2~5μg/L,2组患者不间断地追加维库溴胺。然后比较分析2组患者的临床麻醉效果。结果在对患者行介入神经放射治疗时,通过靶控输注方式注射丙泊酚复合盐酸瑞芬太尼,一方面有利于术中患者血流动力学指标的稳定和知晓病例的降低,另一方面也有利于术后患者的苏醒和拔管。结论与复合静脉泵注方式比较,靶控输注丙泊酚复合盐酸瑞芬太尼对于行介入神经放射学治疗的患者具有更显著的麻醉效果,因此,在临床麻醉过程中可以更进一步推广和使用该靶控输注方式。Objective To investigate the effect of propofol combined with remifentanil on interventional neuroradiology by target-controlled infusion, so as to provide scientific theoretical basis for the effective treatment of disease. Methods One hundred patients treated with interventional neuroradiology therapy in 2013 were consid- ered to be study objects, who were divided into treatment group and control group randomly. And there were 50 cases in each group, respectively. In treatment group, the patients were treated with propofol (2--4 mg/L) com- bined with remifentanil (2--5 μg/L) for anesthesia by target-controlled infusion, while they were treated using propofol combined with remifentanil for intravenous infusion, and the patients in these two groups with uninter- rupted vecuronium bromide for supplement. The anesthesia effect of two groups were compared and observed. Re- sults It was better for the propofol combined with remifentanil by target-controlled infusion in interventional neu- roradiology therapy, because it can not only remain hemodynamic indexes stable and there were no intraoperative awareness during the operation of interventional neuroradiology. What is more, the patients in the postoperative were extubated earlier and waked up faster. Conclusion Compared to propofol combined with remifentanil for in- travenous infusion, it was better for anesthesia effect by using propofol combined with remifentanil on interven- tional neuroradiology with target-controlled infusion. Therefore, this method is worth spreading and using in clin- ic.
