摘  要:目的观察体外冲击波(extracorporeal shock wave,ESW)对兔应力性骨折(stress fracture,SF)中骨骼及局部肌肉组织的影响,探讨ESW治疗与SF修复的关系及作用机制。方法采用发射计算机断层扫描仪(emission computed tomography,ECT)作为诊断依据,建立兔SF模型,将12只造模成功的新西兰兔随机分为两组,冲击波治疗组(C组,n=6)、模型组(B组,n=6),同时设立正常组(A组,n=6)。C组通过ESW治疗,B、A组不进行ESW治疗,冲击波治疗后1、4、8周采用ECT及各组分别处理2只兔子进行病理切片观察,比较3组影像学、骨形态学、肌肉组织变化情况。结果 ECT扫描发现B组兔胫骨放射性计数比值显著高于C组,差异有统计学意义(P<0.05),C组胫骨放射性计数比值与A组比较也有统计学意义(P<0.05)。病理切片显示C组的骨损伤、肌肉损伤较B组明显减轻。结论冲击波可以有效治疗兔下肢SF,促进骨骼、肌肉的修复,可为临床治疗SF提供新的思路。Objective To observe the morphological changes of skeleton and skeletal muscle of rabbits, and to in- vestigate the mechanism of extracorporeal shock wave for the preventing and treating stress fracture.Methods The exper- imental model of stress fracture by emission computed tomography was established, and 12 successful model New Zeal- and rabbits were randomly divided into the treatment group of extracorporeal shock wave (C group,n = 6) and the model group(B group, n = 6) .The blank group contained 6 rabits (A group) .The morphological changes of skeleton and skeletal muscle of rabbits were determined after 1, 4, 8 weeks.Results The radio counting ratio of tibia in the model group was higher than that in treatment group and blank group (P〈0.05) , and there was difference between treatment group and blank group (P〈0.05) .The tibia injury in the model group was more serious than treatment group and blank group.Con- clusion Extracorporeal shock wave can effectively cure stress fracture of tibia in rabbits. This provides a new idea for stress fracture treatment.
