三种敷料应用于供皮区的临床对照研究

摘　　要：目的对照研究三种不同敷料在供皮区的应用,评价其临床应用效果。方法 63名拟行断层皮片移植的患者,平均年龄36.2岁（8~67岁）,男性占71.4%,随机分配到凡士林组、藻酸盐组、含银藻酸盐组,每组21人。供皮区分别用三种不同的敷料处理。对照研究三种不同敷料处理换药时疼痛指数、创面细菌培养阳性率、2周时创面延迟愈合率。结果藻酸盐敷料及含银藻酸盐敷料较凡士林纱布,换药时疼痛指数明显减轻。供皮区细菌培养阳性率,三组间均无差异。2周时延迟愈合率,藻酸盐敷料及含银藻酸盐敷料均较凡士林敷料低（P〈0.05）。藻酸盐敷料与含银藻酸盐敷料两者比较无统计学意义（P〉0.05）。结论藻酸盐敷料较凡士林敷料,有明显的优点,可以作为供皮区标准敷料。在感染预期增加时,可以考虑使用含银藻酸盐敷料。Objective To evaluate the effectiveness of three kind of dressings for management of partial thickness skin graft donor sites.Methods A total of 63 patients,scheduled for partial-thickness skin grafts,which mean age was36.2（8-67）years,71.4% as male,were randomly divided into three groups：vaseline dressings（A）,alginates dressings（B）,and alginates Ag dressings（C）.The donor sites were managed by these three kinds of dressings respectively.Pain intensity,positive rate of bacterial cultivated on donor sites,the rate of delayed healing after the 2 weeks were examined.Results Alginate dressings and Alginate Ag dressing were found to be more comfortable and painless,than Vaseline dressings（P〈0.05）.The positive rate of bacterial cultivated on donor sites there were not difference among three groups.Conclusion Alginate dressings could be used as standard dressings for donor sites.Alginate Ag dressings could be considered on conditions with increasing expectation of infection.
