妊娠期高血压患者血凝指标的变化

摘　　要：目的探究妊娠期高血压患者血凝指标变化的临床意义,为临床提供参考。方法将正常体检和分娩的孕产妇350例按有无合并高血压分为妊娠期高血压组(实验组)75例和正常妊娠晚期孕妇组(对照组)275例,对两组的凝血指标进行检测。结果实验组平均血小板体积(mean platelet volume,MPV)、纤维蛋白原水平(fibrinogen,FIB)、D-二聚体水平高于对照组(t=13.182、10.497、23.880,P=0.000),实验组血小板计数(platelet count,PC)低于对照组(t=2.140,P=0.033),凝血酶原时间(prothrombin time,PT)、部分凝血活酶时间(apartial thromboplastin time,APTT)也少于对照组(t=9.572,P=0.000;t=3.109,P=0.002);轻度子痫前期PC高于重度子痫前期(P<0.05),FIB、D-二聚体水平低于重度子痫前期(P<0.05或0.01);早发型子痫前期的PT少于晚发型子痫前期(P<0.05),FIB、D-二聚体水平高于对照组(P<0.01)。结论妊娠期高血压疾病有明显的高凝现象,且凝血程度与病情严重度有关,有重要临床参考价值。Objective To explore the clinical significance of changes of coagulation indexes in patients with gestational hypertension and make reference to clinic.Methods A total of 350 gravida undergoing normal control group physical examination and childbirth were divided into experimental group with 75 cases with gestational hypertension and 275 normal late pregnancy pregnant women（control group）.The blood coagulation index in two groups were tested.ResultsThe mean platelet volume（MPV）,fibrinogen（FIB）,D-dimer levels of the experimental group were higher than those in the control group（t=13.182,10.497,23.880,P=0.000）.Platelet count（PC）of the experimental group was lower than that in the control group（t=2.140,P=2.140）.Prothrombin time（PT）and a partial thromboplastin time（APTT）of the experimental group was lower than those in the control group（t=9.572,P=0.000;t=3.109,P=0.002）.PC in patients with mild preeclampsia was higher,FIB,D-dimer level was lower than those of patients with severe preeclampsia（P〈0.05 or 0.01）.PT in patients with early-onset preeclampsia was less than those with late-onset preeclampsia（P〈0.05）.FIB,D-dimer levels were higher than the control group（P〈0.01）.Conclusion Patients with gestational hypertension disease has obvionsly higher condensation,and the degree of blood coagulation is associated with disease severity and has important clinical reference value.
