不同剂量酒石酸布托啡诺(诺扬)术后镇痛效应的比较研究

摘　　要：目的：比较不同剂量酒石酸布托啡诺的术后镇痛效应,为临床用药提供参考依据。方法：选择2011年10月~2012年5月于我院进行开腹手术的116例患者作为研究对象,按照随机数字表法分为两组,术后患者均接受自控镇痛,对照组56例采用6mg酒石酸布托啡诺加生理盐水至100ml持续静脉泵入,观察组60例采用8mg酒石酸布托啡诺加生理盐水至100ml持续静脉泵入,比较两组患者术后6、12、24、48h的心率、呼吸频率、血压以及视觉模拟疼痛评分（VAS）情况,观察不良反应发生情况。结果：两组患者术后6、12、24、48h的心率、呼吸频率、血压均无显著性差异（P〉0.05））但观察组患者术后12、24、48h的VAS评分均明显优于对照组,组间有显著性差异（P〈0.05））术后观察组不良反应发生率为5.0%,对照组为7.1%,组间无显著性差异（P〉0.05）。结论：酒石酸布托啡诺用于术后镇痛安全、有效,且8mg用药剂量镇痛效果更佳,可以作为首选的用药方案。Objective：To compare the postoperative analgesic effect of different doses of Butorphanol Tartrate（Butorphanol）,and to provide a reference for clinical.Methods：From October 2011 to May 2012 in our hospital,116 patients of open surgery were selected as the subjects of research,they were divided into two groups according to the random number table and all received controlled analgesia,56 cases of the control group were intravenously pumped with 6mg Butorphanol Tartrate and 100ml Noga saline,60 cases of the observation group were intravenously pumped with 8mg Butorphanol Tartrate and 100ml Noga saline,the heart rate,respiratory rate,blood pressure,and visual analog pain scale（VAS） at 6,12,24,48 h after surgery of two groups were compared,and the occurrence of adverse reactions were observed.Results：There was no significant difference in the heart rate, respiratory rate,blood pressure at 6,12,24,48 h after surgery of two groups（P〉0.05）;the VAS score of the observation group at 12,24,48h after surgery were significantly better than the control group,there was a significant difference（P〈0.05）;the incidence of adverse reactions of observation group was 5.0%,and the control group was 7.1%,there was no significant difference between two groups（P〉0.05）.Conclusion：Butorphanol Tartrate for postoperative analgesia is safety and effective,and 8mg dose has better analgesic effect,it is the preferred regimen.
