肺功能检查在老年性哮喘与慢性阻塞性肺疾病鉴别诊断中的临床应用价值

摘　　要：目的:探讨肺功能检查在老年性哮喘与慢性阻塞性肺疾病(COPD)鉴别诊断中的临床应用价值。方法:选取2010年1月至2013年1月于我院诊断为哮喘或COPD的老年患者69例,其中诊断为单纯哮喘的患者17例(A组)、哮喘合并COPD的患者15例(B组)和单纯COPD的患者37例(C组),所有患者均进行肺功能检查(肺通气功能、肺弥散功能、肺容量指标),观察并分析3组患者肺功能检查结果。结果:患者VC、PEF25、PEF50、PEF75、PEF、MMEF、FEV1/FVC、FEV1/预计值等通气功能指标B、C组明显低于A组,C组明显低于B组,A、B、C组3组间均有显著性差异(P<0.05);患者FRC、RV、RV/TLC等弥散功能指标及DLCO、DLCO/VA等肺容量指标B、C组均明显优于A组,C组明显优于B,A、B、C组3组间均有显著性差异(P<0.05)。结论:肺功能检查在老年性哮喘与COPD临床诊断中具有一定的鉴别价值。Objective：To investigate the lung function test in elderly patients with asthma and chronic obstructive pulmonary disease （COPD） in the differential diagnosis of clinical value. Methods：From 2010 January to 2013 January in our hospital diagnosed as 69 cases of elderly patients with asthma or COPD, diagnosed in 17 patients with asthma（group A）, 15 cases of asthma in patients with COPD（B group） and 37 patients with COPD（C group）, all patients underwent lung function check （pulmonary function, pulmonary function, lung capacity index）, of 3 groups of patients with pulmonary function test results were observed and. Results：Patients with VC, PEF25, PEF50, PEF75, PEF, MMEF, FEV1/FVC, FEV1/estimates, ventilation function indexes B, C group was signiifcantly lower than A group, C group was lower than that in B, A, B, C between the 3 groups were signiifcantly different（P〈0.05）;patients with FRC, RV, RV/TLC etc. the dispersion function and DLCO, DLCO/VA, B, lung capacity index in C group was signiifcantly bet er than the A group, the C group was signiifcantly bet er than B, A, B, C between the 3 groups were signiifcantly different（P〈0.05）. Conclusion：Pulmonary function test has certain value identiifcation in Senile Asthma and COPD in clinical diagnosis.
