双侧切口入路治疗复杂性胫骨平台骨折临床分析

摘　　要：目的：探讨双侧切口入路在复杂胫骨平台骨折手术治疗中的应用及临床效果。方法回顾性分析2009年7月至2012年11月手术治疗的64例复杂胫骨平台骨折患者临床资料，按患者手术方式分为双侧切口入路组及前正中切口入路组，比较两组患者术中情况，术后并发症及膝关节功能恢复情况。结果两组患者手术时间无显著差别，双侧切口入路组术中出血量多于前正中切口入路组，双侧切口组入路术后并发症发生率低于前正中切口组，两组术后12个月膝关节功能优良率无显著差异。结论双侧切口入路治疗复杂胫骨平台骨折有利于减少术后并发症，促进术后膝关节功能的恢复，可依据个人的技术能力在临床中应用。Objective To investigate appling and clinical effect of bilateral approach on operation for complex fracture of tibial plateau. Methods Clinical date of 64 cases patienst who taken operations for complex fracture of tibial plateau from July 2009 to Nov 2012 was analyzed with retrospective way,patients were divided into bilateral approach groups and middle approach groups according to modus operandi,and condition in operation, complication after operations and function of knee joint were contrasted between two groups.Results Time of operation was identical in two groups,volume of blood lossing was more in bilateral approach groups than middle approach groups,rate of complication was lower in bilateral approach groups than middle approach groups, fineness rate of knee joint function was identical between bilateral approach groups and middle approach groups.Conclusion Bilateral approach was heipful for decreasing complication on complex fracture of tibial plateau,improving postoperative recovery of knee joint fuctions,and could be take according private skill in clinic.
