摘 要:目的探讨抗环瓜氨酸肽(抗-CCP)抗体和类风湿因子(RF)在类风湿关节炎(RA)中的诊断价值。方法分别检测抗CCP抗体(酶联免疫吸附实验)、RF及CRP(免疫比浊法)、ESR(魏式法)在RA组、非RA组和健康人中的表达并比较分析。结果 RA组和非RA组CRP、ESR表达水平无显著性差异(P>0.05),而RA组RF表达显著高于非RA组,两者有显著性差异(P<0.01);RA组抗CCP抗体和RF阳性率明显高于非RA组和对照组,数据经统计学比较差异有统计学意义(P<0.01)。抗CCP抗体的敏感性为72.72%,特异性为92.68%;RF的敏感性为86.36%,特异性为85.37%;抗CCP抗体和RF联合检测敏感性为63.64%,特异性为95.12%。结论在RA诊断中,抗-CCP有较高的特异性,RF有较高的敏感性,两者联合检测有利于早期诊断RA。Objective To explore the diagnostic value of anti-cyclic citrullinated peptide (anti-CCP) antibody and rheumatoid factor (RF) in rheumatoid arthritis. Methods Enzyme-linked immunosorbent assay (ELISA) and immuno- turbidimetric assay and Westergrem assay were employed to analyze the expressing levels of anti-CCP antibody, RF, CRP and ESR for RA,non-RA and healthy control groups respectively. Results the expressing levels of CRP and ESR of RA and non-RA groups were not statistically significant (P〉0.05), while the expressing level of RF of RA group was much higher than that of the non-RA group,and the difference was statistically significant (P〈0.01) the positive rate of anti-CCP antibody and RF for RA group was higher than that of the non-RA and control groups,and the differ- ence was statistically significant (P〈0.01). The sensitivity of anti-CCP antibody and RF were 72. 72%and 86.36%, respectively the specificity of anti-CCP antibody and RF were 92.68 % and 85.37%, respectively. The joint-detection sensitivity and specificity for anti-CCP antibody and RF were 63. 640/oo and 95.12,respectively. Conclusion In the diag- nosis of RA, anti-CCP detection is highly specific, RF is highly sensitive,joint detection of anti-CCP and RF is conducive for the early diagnosis of RA.
