摘  要:目的 探讨毒扁豆碱对异氟醚麻醉大鼠认知功能的影响及其机制.方法 2月龄SD大鼠80只,随机分为四组,每组20只.对照组(C组)吸入含有30%氧气的空氧混合气体6 h;异氟醚组(Ⅰ组)吸入1.4%的异氟醚6 h;毒扁豆碱+异氟醚组(PI组)与毒扁豆碱组(P组)腹腔注射毒扁豆碱100 μg/kg,每8小时一次,共3次.首次给药后,PI组吸入1.4%异氟醚6 h;P组吸入同浓度载气6h.麻醉结束行动脉血气分析,12h后取海马组织,ELISA法检测TNF-α、IL-1β、IL-6含量,RT-PCR法检测capase-3 mRNA的表达.光镜下观察海马CA1区锥体细胞形态学变化.麻醉后第2~7天评估认知功能.结果 四组大鼠动脉血气指标、血糖和海马TNF-α含量差异均无统计学意义.与C组和P组比较,Ⅰ组和PI组大鼠海马IL-1β、IL-6含量、capase-3 mRNA明显升高,第4~6天逃避潜伏期明显延长,探索时间明显缩短(P<0.05).与Ⅰ组比较,PI组大鼠海马IL-1β、IL-6含量、caspase-3 mRNA表达明显降低,第4~6天逃避潜伏期明显缩短,探索时间明显延长(P<0.05).结论 毒扁豆碱改善了异氟醚麻醉引起的大鼠认知功能损害,其机制可能与抑制海马炎性反应及神经元凋亡有关.Objective To evaluate the effect of physostigmine on isflurane induced cognition deficit and inflammatory response as well as the expression of caspase-3 in hippocampus in rats. Methods Eighty 2-month-old male Sprague-Dawley rats, weighing 280-320 g, were randomly as- signed into 4 groups (n=20): control group (group C), isoflurane group (group I), physostigmine plus isoflurane group (group PI), and physostigmine group (group P). Rats in group C inhaled a gas mixture of oxygen and air for 6 h and rats in group I inhaled 1.4% isoflurane for 6 h. Physostigmine 100 μg/kg was injected intraperitoneally every 8 h, totally 3 times in the group PI and group P. After the first injection, 1.4% isoflurane was given to the rats in group PI, while same concentration of oxygen and air was given to the rats in group P for 6 h. Blood samples were taken immediately after the end of anesthesia for blood gas analysis. The hippocampus was isolated 12 h after the end anesthesia for determining the expression of TNF-a, IL-1β, IL-6 and easpase-3 mRNA. The pathological change in the pyramid cells of hippoeampus was also examined. The cognitive function was assessed on the 2nd to 7th day after the end of anesthesia. Results There were no significant differences in arterial blood gas analysis, blood glucose level and the expression of TNF-a between four groups. When compared with group C and group P, the expression of IL-1β, and IL-6 in group I and PI were increased, and the expres- sion of caspase-3 in group I and PI were up-regulated (P〈0. 05). The escape latency on day 4-6 in group I and group PI were prolonged, and the probe time was shortened (P〈0. 05). The number of apoptosis cells was increased in group I and PT. When compared with group I, the expression of IL-lβ, IL-6 in group PI were decreased, while the expression of caspase-3 was down-regulated (P〈0.05).
