伯氨喹与蒿甲醚联合治疗恶性疟疾的疗效评价

摘　　要：目的评价伯氨喹与蒿甲醚联合治疗恶性疟疾的疗效。方法选择2013年5月—2014年3月坦桑尼亚中坦友谊医院住院的恶性疟疾100例,按设定的治疗策略随机分为治疗组和对照组各50例。对照组:肌内注射80 mg蒿甲醚,1 d-1,连续治疗7 d,首次剂量160 mg;治疗组:在对照组的基础上,加服22.5 mg伯氨喹,1 d-1,连续服用8 d。依据恶性疟疾临床治愈标准,评价2组患者出院时疗效(痊愈、无效)并观察治疗期间的不良反应和治愈患者出院后28 d内的复发情况。结果治疗组和对照组痊愈、无效、复发率分别为98%、0、2%和82%、4%、14%,差异有统计学意义(P<0.05);总不良反应发生率分别为8%、16%,2组间差异有统计学意义(P<0.05)。结论伯氨喹与蒿甲醚联合治疗可以提高恶性疟疾疗效,降低复发率,不良反应更少。Objective To evaluate the efficacy ofprimaquine and ar temether combined treatment offalcipar um mala r ia. Methods Selected pat ients who were hospit alized i n Si no-Tan zan ia Fr iendsh ip Hospit al with falcipa r u m malaria, collecting in time for the 2013 May to 2014 March, a total of100 cases, among whom 86 were Tanzanian and 14 were Chinese. The patients were divided into 2 groups： 50 cases in the combined group, 50 cases in the control group. For combined therapy group, intramuscular injection of80 mg artemether was used once a day, for 7 consecutive days, the first dose of160 mg, 22.5 mg primaquine was used once a day in the first days oftaking artemether, lasting for 8 days; for the control group treatment： intramuscular injection of80 mg artemether was used once a day for 7 consecutive days, with the first dose of160 mg. The efficacy ofhospital treatment in the two groups（recovery, invalid, reheat） and adverse reaction were evaluated. Results The combined group and the control group is invalid（98%, 0, 2%; 82%, 4%, 14%）, the combined group and the control group, statistically significant differences is invalid, or（P〈0.05）; the incidence ofadverse reaction in combined group and the control group was 8% and 16% respectively, the difference was statistically significant（P〈0.05）. Conclusion Primaquine and artemether combined therapy can improve the curative effect in the treatment offalciparum malaria, and reduce the probability ofrelapse, less adverse reaction.
