多囊卵巢综合征所致复发性流产与体重指数、胰岛素抵抗及疗效相关性研究

摘　　要：目的探讨多囊卵巢综合征(polycystic ovary syndrome,PCOS)所致复发性流产患者与其体重指数、胰岛素抵抗的关系及观察二甲双胍+达英35+耳针+中药治疗胰岛素抵抗患者的疗效。方法本课题将门诊确诊为多囊卵巢综合征所致复发性流产患者150例为研究组,选择非多囊卵巢综合征所致复发性流产患者120例为对照组,比较两组体重指数、胰岛素抵抗指数。研究组根据胰岛素抵抗指数,将存在胰岛素抵抗的患者分为A组50例及B组52例,A组使用二甲双胍+达英35治疗,B组使用二甲双胍+达英35+耳针+中药治疗,两组治疗3月后,使用HMG促排卵治疗,比较A、B组治疗前后体重指数、胰岛素抵抗及流产率。结果研究组与对照组BMI比较,差异有显著性,(P<0.01)、研究组与对照组HOMAR-IR的比较,差异有显著性,(P<0.05),A/B两组治疗后,BMI、HOMAR-IR较治疗前下降,两组治疗后相比差异有显著性,A/B两组治疗后流产率相比差异有显著性。结论 BMI、HOMAR-IR与多囊卵巢综合征(polycystic ovary syndrome,PCOS)所致复发性流产呈正相关关系,使用二甲双胍+达英35+耳针+中药联合治疗,能有效减低体重指数、胰岛素抵抗及流产率。Objective：To discuss the correlation between BMI,IR and recurrent abortion caused by polycystic ovary syndrome,and observe the clinical cure effect of cases with Insulin resistance by therapy of using metformin and diane-35,togher with ear acupuncture and Chinese drug. Method：150 patients with recurrent abortion caused by polycystic ovary syndrome were selected as the study group and 120 patients with recurrent abortion caused by non Polycystic ovary syndrome were selected as the control group,body mass index（BMI）and IR were compared in two groups. In study group,according to IR,Divide 102 cases with Insulin resistance into 2 groups,A group has 50 cases treated with metformin and diane-35,B group has 52 cases treated with Metformin and diane-35,together with ear acupuncture and Chinese drugs,after 3 months treatment,Two groups use HMG for ovulation induction,observe BMI,IR and abortion rates of two groups before and after therapy. Result：The body mass index（BMI）was compared in the study group and the control group,It had marked difference（P〈0.01）. and IR was compared in the study group and the control group,It had marked difference（P〈0.05）. after therapy,the body mass index（BMI）and IR of A and B group were decreased,two had marked difference（P〈0.05）,the abortion rates was compared in A and B group after therapy,there was a marked difference（P〈0.05）. Conclusion：There is a positive between BMI,IR and recurrent abortion caused by polycystic ovary syndrome. The combined treatment of using metformin and diane-35,togher with ear acupuncture and chinese drug can effectively reduce the body mass index,insulin resistance and the rate of abortion.
