摘　　要：目的研究抗甲状腺抗体与女性不孕之间的关系。方法研究观察来我院生殖医学科就诊的不孕女性,空腹采集静脉血检测甲状腺功能及抗甲状腺抗体,纳入甲状腺功能正常者344例,依据抗甲状腺抗体是否阳性分为Ab+组(n=69)与Ab-组(n=275),比较两组患者的年龄,BMI,不孕年限,基础内分泌,月经不规律发生率、子宫内膜异位症发生率、PCOS发生率是否有统计学差异。结果 Ab+组中子宫内膜异位症和PCOS的发生率显著高于对照组(26.09%vs5.09%,P=0.000;24.64%vs 5.82%,P=0.000)。结论 PCOS和子宫内膜异位症常伴有甲状腺自身免疫的异常,子宫内膜异位症和PCOS与TAI相关性不孕可能有共同的免疫遗传病因,而后者又常常导致甲减的发生,为避免母婴不良结局的发生,建议对不孕女性尤其是患有子宫内膜异位症或PCOS的患者筛查甲功(TSH、FT4)和甲状腺自身免疫抗体,必要时予以处理。Objective：Relationship between antithyroid antibodies and female infertility. Methods：Study 344 cases of female infertility whose thyroid function was normal through collecting fasting venous blood from Center for Reproductive Medicine of our hospital,according to whether the antithyroid antibodies was positive,they were divided into two groups,Ab ＋ group（n = 69）and Ab-group（n = 275）,we compared two groups of patients age,BMI,duration of infertility,endocrine basis,the incidence of irregular menstruation,PCOS and endometriosis. Results：the incidence of endometriosis and PCOS of Ab ＋ group was significantly higher than Ab-group（26.09% vs 5.09%,P= 0.000;24.64% vs 5.82%,P = 0.000）. Conclusion：endometriosis,PCOS and TAI-related infertility may have common immune genetic cause,in view of PCOS and endometriosis often accompanied by abnormal thyroid autoimmunity,which in turn often leads to the occurrence of hypothyroidism,so to avoid adverse maternal outcomes,it is recommended,especially for infertile women suffering from endometriosis or PCOS patients,that screen their thyroid function（TSH,FT4）and antithyroid autoantibidies and treat them if necessary.
