急性脑梗死伴高血压患者的心率减速力与心率变异性研究

摘　　要：目的探讨急性脑梗死伴高血压患者的心率减速力(DC)与心率变异性(HRV)。方法选取急性脑梗死伴高血压患者62例,单纯脑梗死患者36例,于发病72 h内行24 h动态心电图监测。选取单纯高血压患者62例、体检正常者80例作对照。脑梗死伴高血压组按梗死部位分为左侧脑梗死组20例,右侧脑梗死组20例,双侧脑梗死组22例。计算机自动计算各组DC值及HRV值。结果与正常组比较,右侧、双侧脑梗死伴高血压组及单纯高血压组DC值显著性降低(P<0.01),各脑梗死组的所有窦性心搏NN间期的标准差(SDNN)、每5 min NN间期平均值的标准差(SDANN)显著性降低(P<0.01)。脑梗死伴高血压组DC与SDNN、SDANN呈中度相关。结论 DC值降低能反映右侧、双侧脑梗死伴高血压患者的迷走神经活性降低,是定量评价迷走神经活性的较好指标。Objective To investigate the deceleration capacity of rate （DC） and heart rate variability （HRV） in patients with acute cerebral infarction with hypertension. Methods An amount of 62 patients with acute cerebral infarction with hypertension and 36 patients with pure acute cerebral infarction and 62 patients with pure hypertension ,80 normal con- trol subjects underwent 24-hour dynamic electrocardiogram recordings . Acute cerebral infarction patients with hypertension were divided into left cerebral infarction group （20 cases）, right cerebral infarction group （20 cases）, bilateral cerebral in- farction group（ 22 cases ） according to the position. DC and HRV values were calculated by computer automatically. Re- suits Compared with the normal group, DC significantly decreased in the right side, bilateral cerebral infarction with h^- pertension groups and pure hypertension group, standard deviation of NN intervals （SDNN）, standard deviation of the aver- ages of NN （SDANN） decreased significantly in each cerebral infarction group（ P〈0.01 ）. DC was correlated with SDNN, SDANN in hypertension groups with cerebral infarction. Condnsion Low DC values could reflect reduced vagal activity in patients of right, bilateral cerebral infarction with hypertention. DC could be a better index as a quantitative evaluation of vagal activity.
