摘 要:目的 研究载脂蛋白E(ApoE)基因多态性与高脂血症发病的关系,为高脂血症的预防控制提供循证医学的证据。方法 查阅在中国学术期刊全文数据库(CNKI)、中国万方和维普数据库上收录的1990-2014年有关我国高脂血症人群与ApoE基因多态性研究的相关文献,利用Meta分析对研究结果进行数据合并,计算合并OR值及95%CI,并评价结果稳定性和可靠性。结果 共纳入11个病例对照研究的文献,Meta分析结果表明,以野生型E3/3基因型为参照,携带E3/4,E4/4基因型的人群发生高脂血症的危险性的OR值和95%可信区间分别为1.827(1.433~2.329)、3.030(1.413~6.498),携带ε4等位基因的OR值和95%可信区间为1.730(1.400~2.510)。异质性检验和敏感性分析结果说明纳入文献偏倚性较小,研究结果可靠。结论 ApoE基因多态性与高脂血症密切相关,携带ε4等位基因的人群具有发生高脂血症的高危险性。Objective To discuss the association of apolipoprotein E (ApoE) gene polymorphisrn with hyper lipidemia, and provide evidence for the prevention and control of hyperlipidemia. Methods All related literatures were retrieved from 1990 to 2014 in CNKI, WanFang and VIP database and analyzed by recta-analysis. The pooled OR and its 95%CI were calculated and the reliability and stability of the recta-analysis results were evaluated. Results Eleven case-control studies were selected and analyzed. Meta-analysis showed that the pooled OR values (95 %CI) of hyperlipidemia among subjects with genotype E3/4, E4/4 or with at least one e4 allele were 1. 827 (1. 433-2. 329), 3. 030 (1. 413-6. 498) and 1. 730 (1. 400-2. 510), respective- ly. Heterogeneity testing and sensitivity analysis revealed that the bias was small and the results were reliable. Conclusions The polymorphism of ApoE is significantly associated with hyperlipidemia. Individuals harho ring ε4 allele usually face higher risk of hyperlipidemia than others.
