超声探查胎儿颈项透明层厚度在产前筛查中的应用价值

摘  要:目的:探讨孕早期胎儿颈项透明层(NT)在产前超声筛查胎儿畸形中的价值。方法:对2010年6月1日-2011年7月31日期间于我院检查的11~13咱孕周单胎妊娠孕妇经腹部超声测量胎儿头臀径(crown-rump length,简称CRL)长度,推算超声孕周后在胎儿正中矢状切面连续3次测量NT,记录NT最大值并跟踪随访妊娠及生育全程。旨在探讨超声测定颈项透明层对诊断胎儿畸形的临床意义。结果:3377例孕妇中发现胎儿颈项透明层增厚者36例。NT值〉2.5mm。26例胎儿有先天性异常,其中无脑儿1例,颈部淋巴水瘤9例,泌尿系统畸形2例,消化系统畸形1例,胸膜腔积液2例,内脏外翻3例,脉络丛囊肿3例,全身皮肤水肿2例,26例中合并死胎3例。结论:超声测量NT的最佳时间应该是在11~13均孕周,CRL值在45mm~84mm之间。NT增厚可作为早期筛查胎儿畸形的一个重要指标,而且无创伤,适于各年龄的孕妇,在胎儿产前畸形筛查中具有重要临床应用价值。Objective To study the early pregnancy fetal neck transparent layer (NT) in the value of prenatal ultrasound screening for fetal malformation Methods From June 1, 2010 to July 31, 2011 the hospital check during the period of 11 ~ 13 +6 weeks pregnancy single pregnancy pregnant women after abdominal ultrasound measurement of fetal head hip size (the crown to rump length, referred to as the CRL) length, calculate the fetus after ultrasonic gestational age median sagittal section 3 consecutive measurement of NT, record a maximum NT and pregnancy and birth All the follow-up ultrasound was m explore neck transparent layer of the clinical significance of the diagnosis of fetal abnormalities Results Neck transparent layer thickening of 36 cases in 3377 cases of pregnant women were found in fetal. NT value 〉 2.5 mrrL 26 cases with congenital fetus abnormalities, including I case of anencephaly,, neck lymphatic tumor 9 cases, urinary a-act malformation in 2 cases, the digestive system malformation in I case and pleural effusion in 2 cases, visceral valgus in 3, chomid plexus cyst in 3, the whole body skin edema in 2 cases, 26 cases merger stillbirth in 3 cases. Condusion The ultrasonic measurement should be the best time of NT in 11 ~ 13 +6 gestational age, the CRL value between 45 mm to 84 mm NT thickening can be used as an important index of early screening for fetal abnormalities, and no Waunma suitable for all age of pregnant women, prenatal fetal malformation screening has the important dinical value.
