摘　　要：目的：探讨连续性静脉一静脉血液透析滤过（CVVH）对重症急性胰腺炎（SAP）合并急性呼吸窘迫综合征患者的疗效。方法：2007年2月-2014年4月收治24例重症急性胰腺炎合并急性呼吸窘迫综合征患者，随机分为对照组12例，治疗组12例。治疗组在对照组治疗基础上行CVVH治疗，均采用中心静脉置管建立血管通路，合采用普通肝素或低分子肝素抗凝，每天治疗时间8～12h。观察患者行HRRT后生化指标、APACHEII评分的变化以及临床转归。结果：与对照组比较，经CVVH治疗后，氧合指数、WBC、CRP、AST、ALP、Cr、BUN均显著降低（P〈0．05）；APACHEII评分降低、住院天数缩短、治愈好转率均显著升高，具有统计学意义（P〈0．05）。结论：CVVH可能是治疗重症急性胰腺炎（SAP）合并急性呼吸窘迫综合征的—种有效的辅助治疗方法，能降低患者的病死率，提高治愈好转率。Objective Continuous venous blood dialysis （CWH） on severe acute pancreatitis （SAP） and the curative effect of patients with acute respiratory distress syndrome.
