摘  要:目的:对初诊的Ⅱ型糖尿病患者进行短期胰岛素强化治疗,探讨其对Ⅱ型糖尿病患者胰岛D细胞功能的影响。方法:将35名初诊Ⅱ型糖尿病患者,进行短期胰岛素泵强化治疗。观察治疗前后空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbAlc)、空腹及餐舌胰岛素、C肽变化,HOMA胰岛素抵抗指数(HOMA—IR)和HOMA-β细胞功能指数(HOMA-β)。结果:1.撤泵次日FPG、2hPG、HOMA-IR均较治疗前减低,FC—P、2hC-P、HOMA—β较治疗前有所增高,P〈0.05,差异有统计学意义;2.BMI、TG、TC、LDL-C与胰岛素抵抗均存在正相关,且BMI的r值最大,相关性最强。结论:1.对初诊II型糖尿病患者进行短期胰岛素泵强化治疗,可快速解除葡萄糖毒性,减轻胰岛素抵抗,改善胰岛p细胞功能;2.BMI、TG、TC、LDL-C与胰岛素抵抗存在正相关,其中BMI相关性最强,BMI、血脂异常可能是导致胰岛素抵抗的较为重要的因素。Objective The new type Ⅱdiabetes of short-term insulin intensive treatment, discuss its Ⅱ islet beta cell function in patients with diabetes. Methods 35 new II with type 1 diabetes, short-term insulin pump intensive treatment. Were observed before and after treatment of fasting plasma glucose (FPG), 2h postprandial blood glucose (2 HPG), glycosylated hemoglobin (HbAlc), fasting and postprandial insulin, C peptide, HOMA insulin resistance index (HOMA IR) and HOMA - beta cell function index (HOMA - beta). Results 1. The day after off pump FPG, 2 HPG, HOMA - IR was reduced, the FC - E 2 hc - P, HOMA - beta is higher before treatment, P 〈 0.05, think that the difference was statistically significant; 2. The BMI, TG, TC, LDL - C there were positive correlation with insulin resistance, and BMI value of r is the largest, most relevant. Conclusion 1. The first diagnosis II short-term insulin pump intensive treatment with type 1 diabetes, can quickly remove glucose toxicity, reduce insulin resistance, improve the function of islet beta cells; 2. The BMI, TG, TC, LDL - C is positively correlated with insulin resistance, the strongest correlation with BMI, BMI, dyslipidemia may be more important factors lead to insulin resistance.
