摘　　要：成功的支付制度改革可以赋予公立医院加强内控的原动力,倒逼公立医院进行改革。支付制度对公立医院实行契约化管理,当公立医院在约定的服务价格下,无法实现既定目标、获得应有回报时,可能会用抗拒、阻碍的方法来应对,反过来影响支付制度改革。支付制度改革成功后,公立医院则应以提高社会效率为指导,加强宏观和微观统筹管理,进行回归公益性的改革。Successful payment system reform can give the motive force of the public hospitals to strengthen internal control, and push them to reform. The way of insurance payment makes a contract management of public hospital. However, when the public hospitals cannot achieve their goals and get their payment based on the contract service price, it will cause resisting or obstructing in health care, which in turn affect payment reform. After the successful reform of insurance payment, the public hospital should aim to improve their social efficiency, strengthen the macroscopic and microscopic overall management, and regress to their role of public welfare.
