不同听力曲线突发性耳聋患者血液流变学及治疗效果分析

摘　　要：目的探讨不同听力曲线突发性耳聋患者的血液流变学改变情况及治疗效果。方法采集本院2010年3月~2011年10月收治的74例突发性耳聋患者的病史资料,所有患者给予血液流变学检测及治疗前后纯音测听,对所得结果进行统计学分析。结果 1（平坦型组）、2组（高频下降型组）的全血黏度、血浆黏度差异无统计学意义（Q=0.53、0.08、0.42、0.00,P〉0.05）;1、2组的全血黏度、血浆黏度均高于3组（Q=3.97、4.25、3.47、4.58,P〈0.05;Q=3.86、3.62、3.33、3.97,P〈0.01）。1、2、3组的总有效率分别为57.14%、66.67%、100.00%,1、2组的治疗效果差异无统计学意义（χ2=0.48,P〉0.05）;1、2组的总有效率与3组比较,差异有统计学意义,3组的疗效优于1、2组（χ2=8.84、5.74,P〈0.05）。结论听力曲线为平坦型及高频下降型的突发性耳聋患者,其全血黏度及血浆黏度高于低频下降型者。低频下降型组的治疗效果优于其余两组。Objective To explore the change of hemorheology and therapeutic effect on patients with sudden deafness in different audiometric curve. Methods Medical history of 74 patients with sudden deafness treated in our hospital from March 2010 to October 2011 were collected,hemorheology was determined and pure tone test before and after treatment was carried out in all patients. Results There was no statistical difference of whole blood viscosity or plasma viscosity between group 1（flat-type group） and group 2（high-frequency descending group）（Q=0.53,0.08,0.42,0.00,P〈0.05）.The whole blood viscosity and plasma viscosity in group 1 and group 2 were all lower than those in group 3（Q=3.97,4.25,3.47 and 4.58,P〈0.05;3.86,3.62,3.33 and 3.97,P〈0.01）.The total effective rate of group 1,2 and 3 was 57.14%,66.67% and 100% respectively.There was no statistical difference in curative effect in group 1 and 2（χ2=0.48,P〈0.05）.There was a statistical difference of the total effective rate in group 1 and 2 compared with group 3,the curative effect of group 3 was better than that of group 1 and 2（χ2=8.84 and 5.74,P〈0.05）. Conclusion The whole blood viscosity and plasma viscosity are higher in patients with sudden deafness whose audiometric curve types belonging to flat-type and high-frequency descending type in comparison with those in low-frequency descending type.The therapeutic effect in low-frequency descending type group is superior to that in the other two groups.
