自拟活血通络止痛汤加减治疗带状疱疹后遗肋间神经痛的临床观察

摘　　要：目的观察自拟活血通络止痛汤加减治疗带状疱疹后遗肋间神经痛(PHN)的疗效。方法将84例PHN患者随机分为观察组及对照组,两组各42例。对照组给予内服维生素B1、盐酸多塞平片、甲钴胺片加外用1%双氯芬酸钠乳剂等常规治疗,观察组在常规治疗的基础上给予自拟活血通络止痛汤加减治疗。比较治疗后1周、2周、4周的数字疼痛评分量表(NRS)、皮肤病生活质量指数评分(DLQI)、匹兹堡睡眠质量评分(PSQI)及抑郁评分(SDS评分),同时比较24 h疼痛持续时间及24 h总睡眠时间、疼痛缓解有效率。结果治疗后1周、2周、4周,两组的NRS评分、DLQI评分、PSQI评分及SDS评分均较治疗前降低(P<0.05),观察组各项评分均低于对照组(P<0.05);治疗后各时间点观察组的24 h疼痛持续时间明显少于对照组(P<0.05),而24 h总睡眠时间长于对照组(P<0.05);治疗后各时间点观察组的疼痛缓解有效率明显高于对照组(P<0.05)。结论自拟活血通络止痛汤加减治疗带状疱疹后遗肋间神经痛的临床效果显著,能够明显改善患者的疼痛及抑郁症状,提高生活质量,可取的满意的综合疗效。Objective To observe the clinical effect of the method of Modified promoting Qi activating blood,dredging the collaterals to stop pain on the postherpetic neuralgia（ PHN）. Methods 84 patients with postherpetic neuralgia in this experiment totally were divided into treatment group and control group randomly. It has 42 cases per group. Control group accepted treatment with oral Vitamin B1,Mecobalamin and external Doxepin and Diclofenac Sodium Cream. Treatment group accepted the same with control group and plus the method of Modified promoting Qi activating blood,dredging the collaterals. The score changes of the indexes of patients after treatments were observd for one week,two weeks and four weeks later,which included the scores of pain degree（ NRS）,sleep quality（ PSQI）,dermatology life quality index（ DLQI） and self-rating depression scale（ SDS）. Results After treatment 1 week,2 weeks,4 weeks,the results of NRS,DLQI,PSQI and SDS in the observation groupwere significantly lower than the control group（ P〈0. 05） in. After treatment,24 h duration of pain in the observation group at each time point was significantly less than the control group（ P〈0. 05）,and 24 h total sleep time was longer than the control group（ P〈0. 05）; each time point efficiency of pain relief in observation group was significantly higher than the control group（ P〈0. 05）. Conclusion The method of modified promoting Qi activating blood,dredging the collaterals to stop pain has good effect on treating postherpetic neuralgia. It can obviously reduce the pain degree,improve the sleeping quality and the ability of work or housekeeping work,and enhance the quality of their life.
