急性冠脉综合征患者血清五聚素3水平及与近期预后的相关性研究

摘　　要：目的探讨急性冠脉综合征(ACS)患者血清五聚素3(PTX3)的水平及对近期预后判断的意义。方法随机选取2013年12月至2014年8月心内科住院的冠心病患者156例,其中稳定性心绞痛(SA)患者38例,ACS患者118例,包括不稳定性心绞痛(UAP)患者64例,急性心肌梗死(AMI)患者54例;采用ELISA法检测各组入院时血清PTX3水平及ACS组患者正规治疗7天后血清PTX3水平。在出院6月后对患者进行随访,了解患者近期预后,对PTX3水平与预后做相关性分析。结果 ACS组患者PTX3水平显著高于SA组患者(SA组:874.56±128.25 pg/ml,UAP组:1354.74±254.94 pg/ml,AMI组:1 474.27±278.18 pg/ml),(P<0.05);ACS组7天后PTX3水平较入院时明显降低(UAP组入院时:1 354.74±254.94 pg/ml,7天后:1 035.37±249.13 pg/ml,AMI组入院时:1 474.27±278.18 pg/ml,7天后:1 067.35±264.58 pg/ml)(P<0.05);血清PTX3、hs-CRP、LDL-C与ACS患者近期预后相关(P<0.05)。结论 ACS患者血清PTX3表达升高,且提示近期预后不良。Objective To explore the relationship between serum level of Pentraxin 3 and short- term prognosis in patients with acute coronary syndrome（ ACS）. Methods A total of 156 patients with coronary artery disease including 38 patients with stable angina pectoris（ SAP）,64 patients with unstable angina pectoris（ UAP） and 54 patients with acute myocardial infarction（ AMI） were enrolled in this study. Serum level of PTX3 was detected by ELISA at admission and after treatment for one week in ACS group. The short- term prognosis of patients with ACS had been followed up by phone at 6 months later. Results The serum level of TPX3 in ACS group was significantly higher than that of SAP group（ SAP group： 874. 56 ± 128. 25 pg / ml）,UAP group（ 1 354. 74 ± 254. 94 pg / ml）,and AMI group（ 1 474. 27 ± 278. 18 pg / ml）（ P〈0. 05）. The serum level of TPX3 in ACS group was significantly decreased after 7 days than that at admission（ UAP group at admission： 1 354. 74 ± 254. 94 pg / ml,7days later： 1 035. 37 ± 249. 13 pg / ml）,and AMI group at admission：（ 1 474. 27 ± 278. 18 pg / ml,7 days later： 1 067. 35 ± 264. 58 pg / ml）（ P〈0. 05）. The short- term prognosis of patients with ACS was closely correlated with serum levels of PTX3,hs- CRP and LDL- C（ P〈0. 05）.Conclusion Serum level of PTX3 is significantly elevated in patients with ACS,and the increased serum level of PTX3 indicates poor short- term prognosis in patients with ACS.
