摘　　要：目的:探索单一使用利培酮、阿立哌唑或氯氮平治疗精神分裂症对肝功能的影响。方法:用历史性队列研究的方法,评估110例精神分裂症患者单一使用利培酮、阿立哌唑或氯氮平治疗精神分裂症对谷丙转氨酶(ALT)及谷草转氨酶(AST)的影响,并且对这些患者的临床处理和转归进行随访。结果:在110例患者中,共有29例患者发生转氨酶异常,占总人数的26.4%;其中氯氮平组13例(33.3%),利培酮组14例(35.9%),阿立哌唑组2例(6.3%),氯氮平组及利培酮组出现谷丙转氨酶(ALT)及谷草转氨酶(AST)增高的程度相对阿立哌唑组严重,多需要药物干预治疗,阿立哌唑组程度较轻。结论:氯氮平及利培酮治疗精神分裂症时会出现较为严重的肝功能异常,阿立哌唑相对较轻。Objective： To explore the effects of the atypical antipsychotic drugs risperidone, clozapine and aripiprazole on liver function in schizophrenic patients. Methods： In this retrospective cohort study, we screened out 110 patients treated with risperidone, clozapine or aripiprazole alone from 8531 schizophrenic patients. We evaluated the changes in ALT and AST after antipsychotic treatment and followed up the management and clinical outcome. Results： In the 110 patients, abnormal liver function occurred in 29 patients. In aripiprazole group, ab- normal liver function occurred transiently and mildly in 2 patients （6.3%） ; in risperidone or clozapine group, more severe abnormal liver function occurred in 14 patients （35.9%） or 13 patients （33.3%）. Conclusion： Risperidone or elozapine may cause more severe abnormal liver function, all of which could restore to normal after treatment. Aripiprazole-caused abnormal liver function is mild and transient.
