腹腔镜联合胆道镜治疗胆囊息肉合并胆囊结石78例疗效观察

摘　　要：目的探讨腹腔镜联合胆道镜治疗胆囊息肉合并胆囊结石的临床效果。方法选取2012年1月至2014年4月我院收治的78例胆囊息肉合并胆囊结石患者作为研究对象,并将其随机分为研究组和对照组各39例,研究组采用腹腔镜联合胆道镜治疗,对照组采用开腹手术治疗,随访6个月,综合比较两组患者的手术时间、术中出血量、肛门排气时间、住院时间和并发症发生情况。结果两组患者手术时间相比差异无统计学意义(P>0.05);研究组术中出血量、肛门排气时间、住院时间均明显少于对照组,差异有统计学意义(P<0.05);研究组并发症发生率为2.56%,显著低于对照组的20.51%,差异有统计学意义(P<0.05)。结论腹腔镜联合胆道镜治疗胆囊息肉合并胆囊结石效果确切,具有创伤小、恢复快及并发症少等优势,值得临床推广应用。Objective To investigate the clinical effect of laparoscope combined with choledochoscope in the treatment of patients with gallbladder polyps complicating cholecystolithiasis. Methods 78 cases of patients with gallbladder polyps complicating gallstone treated in our hospital from January 2012 to April 2014 were selected and randomly divided into research group and control group, with 39 cases in each group. The research group was treated with laparoscopy combined with choledochoscopy, while the control group was treated with traditional laparotomy. All patients were followed up for six months. The operative time, bleeding volume, anus exhaust time, hospitalization time and complications of two groups were compared. Results The operative time of two groups had no statistically significant difference （P〉0.05）;the bleeding volume, anus exhaust time and hospitalization time of research group were less than those of control group, with statistical difference （P〈0.05）;the incidence of complications of research group was 2.56%, significantly lower than 20.51%of control group, with statistical difference （P〈0.05）. Conclusions Laparoscope combined with choledochoscope has exact effect in the treatment of gallbladder polyps complicating cholecystolithiasis, with mild trauma, rapid recovery and less complications, which deserves promotion in clinic.
