无创正压机械通气联合雾化吸入治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者的临床效果

摘　　要：目的探讨无创正压机械通气(BiPAP)联合雾化吸入治疗稳定期重度慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者的临床效果。方法选取我院收治的稳定期重度COPD合并Ⅱ型呼吸衰竭患者90例,将患者分为观察组和对照组,各45例,对照组患者给予吸氧、止咳祛痰、抗感染、补液等常规治疗,观察组患者在此基础上给予BiPAP联合雾化吸入治疗,比较两组患者治疗前后的血气分析情况以及肺功能情况。结果治疗后两组患者的血气分析、肺功能指标均得到明显改善,且观察组患者改善情况更明显,差异均有统计学意义(均P<0.05)。结论应用Bi PAP联合雾化吸入治疗稳定期重度COPD合并Ⅱ型呼吸衰竭患者可提高治疗效果,可改善患者的临床症状以及动脉血气指标。Objective Noninvasive positive pressure mechanical ventilation combined atomization inhalation in the treatment of stable stage of chronic obstructive pulmonary disease （COPD） with typeⅡ respiratory failure effect. Methods Stable period in our hospital were selected 90 patients with severe COPD patients with typeⅡ respiratory failure,45 cases each.The patients were divided into observation group and control group,the control group was given oxygen inhalation,cough expectorant,anti infection,fluid replacement and other conventional treatment,the observation group based on the giving of noninvasive positive pressure mechanical ventilation combined atomization inhalation treatment,blood gas analysis and pulmonary function were compared before and after treatment in two groups.Results After treatment, blood gas analysis of two groups of patients with pulmonary function indices were obviously improved,and the observation group to improve the situation is more obvious,the difference had statistical significance（P〈0.05）.Conclusion Application of BiPAP combined atomization inhalation in the treatment of stable severe COPD patients with typeⅡ respiratory failure can improve the treatment effect,can improve the clinical symptom,arterial blood gas index.
