医护人员主导的家庭会议在1例晚期癌症患者中的应用

摘　　要：癌症患者的病情告知和临终前医疗护理决策的制定,是医护人员和家属面临的难题。本科室对1例晚期结肠低分化腺癌临终女性患者,为其在医院召开了3次家庭会议,由医护人员主导。通过召开家庭会议,告知患者的病情,征求患者及家属对于医疗护理的意见,在充分尊重患者及家属的基础上制定出合理的医疗护理决策;在会议的过程中,向患者及其家属提供精神心理方面的支持,引导家属与患者的情感沟通与表达,帮助患者整合社会支持系统,让患者获得更多的支持与鼓励,提高患者临终阶段的生活质量和死亡质量。认为在会议召开前,充分的准备,以及医护人员较好的沟通协调能力、应对死亡的专业知识和技能,良好的医护患关系,能够促进家庭会议的成功召开,并达到期望的效果。One of the difficulties medical staff and families of cancer patients was disease informing and working out effective nursing strategy for patients before death. Three medical staff-led family conferences were held for one dying female patients with colon poorly differentiated adenocarcinoma in the department. During the conferences, the patient was told about the disease then flexible nursing strategy was designed in accordance with the patient ＇s and families＇ suggestions. Meanwhile, psychological support was provided to help conduct communication between the patient and families and medical staff help the patient obtain more social support to improve the quality of life and death. We regarded that adequate preparation, good communication skills, specialized knowledge of medical staff and good relationship between patients and nurses contribute to the success of family conference.
