护士对经外周穿刺中心静脉导管置管前评估的认知与实践的现状

摘　　要：目的了解临床护士对经外周穿刺中心静脉导管(peripherally inserted central catheter,PICC)置管前评估的认知以及实践现状,以期为护理管理者及临床护士规范PICC置管前评估提供参考依据。方法 2014年4月,便利抽样法选择参加安徽省输液治疗护理进展培训班的201名护理人员为研究对象,采用自行设计的调查问卷对其进行PICC置管前评估认知与实践的现况调查。结果 100%护士认为PICC置管前评估有必要,其中89.1%护士认为PICC置管前评估非常有必要。有87.1%的护士报告其所在的医院在PICC置管前进行评估,但只有21.9%的护士非常了解置管前评估内容。评估内容中,血管条件、治疗周期、药物性质的评估率均达90%以上。记录评估结果的占71.1%,且记录形式不一。52.2%的护士是根据自己平时学习或参加培训班获得的知识进行评估。只有9.6%的护士知道复旦大学护理学院构建了PICC置管前评估实践指南及评估表。结论临床护士PICC置管前的认知与实践情况不够理想,缺乏规范统一的评估内容、评估标准及记录形式。护理管理者应统一置管前评估标准,加大PICC置管前评估培训力度,为PICC规范置管提供质量保障。Objective To explore the current situation of the clinical nurses’cognition and practice to-wards assessment before PICC placement,and to provide the references for nurse managers and clinical nurses to standardize the procedure.Methods By convenience sampling method,201 nursing staffs were selected and we investigated their cognition and practice towards assessment before the PICC catheterization by using self-designed questionnaire.Results 100% nurses perceived that it was necessary to do the assess-ment before the PICC catheterization,of which 89.1% of them thought that it is quite necessary.87.1%nur-ses reported that assessment before the PICC placement was conducted in their hospitals,however,only 21 .9% nurses were familiar with the content of the assessment.Among the content of the assessment,the evaluating rate of blood vessel condition,treatment cycle and drug properties was above 90%.Additionally, 71.1% nurses recorded the evaluation results with various forms.52.2% nurses were acquired the knowl-edge form self study or take training lessons.Only 9.6% nurses knew that Fudan University developed a practice manual and evaluation sheet of assessment before the PICC placement.Conclusion The survey in-dicates the unsatisfying situation of the clinical-nurse’s evaluating cognition and practice before the PICC catheterization,with lacking of standard and uniform evaluation content,evaluation criterion and record form.Nursing managers should unify the evaluation criterion,reinforce the evaluation training,and thus to ensure the standardization of the evaluation before the PICC catheterization.
