摘 要:目的:通过观察米力农对心力衰竭大鼠心肌钙离子(Ca2+)、基质金属蛋白酶-2(MMP-2)、肌钙蛋白酶Ⅰ(c TnⅠ)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)的影响,探讨米力农通过细胞内钙调节治疗心力衰竭的机制。方法:将30只健康雄性SD大鼠随机分为对照组、心力衰竭组、米力农组各10只,对照组腹腔注射生理盐水0.75 m L/kg,每周2次,共7周;心力衰竭组、米力农组建立心力衰竭模型:盐酸阿霉素用生理盐水配成2 mg/m L的注射液,以0.75 m L/kg(1.5 mg/kg)腹腔注射,每周2次,共7周;米力农组在第7周给予米力农50μg/kg,加注射用水配制成0.5 m L的注射液经腹腔注射,每天1次,共7 d。分别在末次给药后24 h检测三组大鼠心肌Ca2+、血清MMP-2、c TnⅠ及心肌酶CPK、CK-MB水平。结果:心力衰竭组心肌Ca2+及血清MMP-2、c TnⅠ、CPK、CK-MB水平均比对照组(P均<0.01)和米力农组高(P<0.05)。结论:心力衰竭时心肌肌浆网钙泵活性受损,米力农可有效促进Ca2+内流和肌浆网钙的释放产生正性肌力作用,从而改善心脏功能,减轻心肌受损。Objective: To research the changes of myocardial calcium ions (Ca2+), matrix metalloproteinase-2 (MMP-2) and troponin-I (cTnI), creatiue kinase (CK), creatine kinase isoenzyme (CK-MB) of heart failure rat with milrinone, and investigate the mechanism of milrinone in treating heart failure by regulating intracellular calcium. Methods: Thirty rats were divided into control group, heart failure group, milrinone group. The control group was given saline solution 0.75 mL/kg by intraperitoneal injection, 2 times a week, a total of 7 weeks. Rats of heart failure group and milrinone group were builded model of heart failure. Hydrochloric acid doxorubicin mixed with physiological saline injection into 2 mg/mL, the rats of heart failure model were given 0.75 mL/kg ( 1.5 mg/kg) by intraperitoneal injection, 2 times a week, a total of 7 weeks; Milrinone (50μg/kg) plus injection water to mixture injection, milrinone group were given 0.5 mL mixture injection by intraperitoneal injection, once a day, a total of 7 days. Detected the levels of Ca^2+, MMP-2, cTnI and myocardial enzymes CPK, CK-MB 24 hours after the last administration. Results: Compared with the control group, the levels of Ca2+, MMP-2, cTnI, CPK, CK-MB were increased in the heart failure group ( P〈0.01 ) and milrinone group (P〈0. 05 ). Conclusion: Activity of myocardial sarcoplasmic reticulum calcium pump is impaired in heart failure, Milrinone can effectively promote calcium influx and sarcoplasmic reticulum calcium release to produce a positive inotropic effect, relieve heart failure.
