摘 要:目的:探讨丙氨酰谷氨酰胺(Ala-Gln)对机械通气极低出生体质量儿是否具有肺、肠的保护作用和免疫调节作用。方法:回顾性分析2013年1-12月我院治疗痊愈出院的39例机械通气极低出生体质量儿的临床资料,其中使用Ala-Gln 24例(Ala-Gln组),未使用Ala-Gln 15例(对照组)。比较分析两组患儿在撤机时间、开奶时间、到达正常奶量时间、平均每日体质量增长以及发生喂养不耐受、胆汁淤积、坏死性小肠结肠炎的差异性,并观察Ala-Gln组有无明显不良反应发生。结果:与对照组相比,Ala-Gln组撤机时间、开奶时间、到达正常奶量时间明显缩短,体质量增长快,喂养不耐受、胆汁淤积、坏死性小肠结肠炎发生率明显降低(P均<0.01)。Ala-Gln治疗过程中未见明显不良反应。结论:Ala-Gln对机械通气极低出生体质量儿可能具有肺、肠保护作用和免疫调节作用。Objective: To investigate the effect of protection and immune regulation of alanyl-glutamine (Ala-Gln) in lung and intestine of very low birth weight infants with mechanical ventilation. Methods: Thirty-nine very low birth weight infants with mechanical ventilation recovered from January to December 2013 were analyzed retrospectively, including the use of Ala-Gln in 24 cases ( Ala-Gln group), without Ala-Gln 15 cases (control group). To investigate differences of two groups in weaning respirator time, open milk time, reaching the normal milk time, the average daily weight gain and feeding intolerance, cholestasis, neerotizing enterocolitis and obvious adverse reaction in Ala-Gln group. Results: Compared with Ala-Gln group and control group, weaning respirator time, open milk time,reaching the normal milk time were shortened obviously, weight fast growth, incidence rate of feeding intolerance, cholestasis and necrotizing enterocolitis were decreased evidently, there were significant difference (P〈0.01). Conclusion: Ala-Gln on very low birth weight infants with mechanical ventilation has the effect of protection and immune regulation probably in lung and intestine. It has no obvious adverse reaction in Ala-Gln group.
