振动排痰机治疗慢性阻塞性肺疾病急性加重期临床观察

摘　　要：目的观察振动排痰机治疗慢性阻塞性肺疾病急性加重期(AECOPD)的临床疗效。方法128例AECOPD患者,随机分为观察组和对照组,各64例,两组均给予抗炎、解痉平喘、止咳化痰、雾化吸入、持续低流量吸氧或无创呼吸机辅助呼吸等常规治疗,对照组加用人工扣背排痰法,观察组加用体外振动排痰机行机械叩振排痰法。结果 1观察组较对照组HR、RR、Pa CO2、p H值下降更明显,Pa O2、Sa O2上升更明显,住院天数缩短,抗生素使用天数缩短,24小时排痰量增加,差异均具有统计学意义(P<0.01)。2观察组患者的气急、胸闷、呼吸困难改善,呼吸功能较对照组明显改善。3观察组湿啰音消失率84.4%(54/64)与对照组67.2%(43/64)相比差异有统计学意义(P<0.05)。结论体外振动排痰机治疗AECOPD患者具有显著的排痰效果,能改善患者的血气与肺功能状态,具有操作简单、容易掌握、痛苦小、缩短疗程,住院时间短等优点,是治疗AECOPD的一种安全有效的方法。Objective To observe the curative effect of vibration row phlegm machine in the treatment of patients with AECOPD. Methods 128 AECOPD patients were randomly divided into the observation group and the control group, 64 cases in each group. All patients were given conventional treatment of anti inflammation, spasm and asthma, cough and phlegm, inhalation, persistent low flow oxygen inhalation or noninvasive ventilator assisted breathing. The control group was treated with manual button back expectoration, and the observation group was given external vibration row phlegm machine mechanical percussion vibration sputum elimination method. Results ①The value of HR, RR, PaCO2 and pH decreased more significantly in the observation group than in the control group, and the level of PaO2 and SaO2 increased more significantly in the observation group than in the control group. The duration of hospital stay and the days of antibiotic use were lower in the observation group than in the control group, and 24 hours sputum volume was higher in the observation group than in the control group （P〈0. 01）. ②The improvement of shortness of breath, chest tightness, dyspnea and respiratory function was more pronounced in the observation group than in the control group. ③ The rate of moist rales disappearance was 84. 4% （54/64） in the observation group and 67. 2% in the control group （43/64） （P〈0. 05）. Conclusion Extrasomatic vibration sputum elimination machine can improve the blood gas and pulmonary function of AECOPD patients, with the advantages of simple operation, easy to master and short duration of hospital stay.
