纤支镜灌洗与常规治疗重症肺部感染合并肺不张137例对比分析

摘　　要：目的探讨经纤维支气管镜支气管肺泡灌洗治疗重症肺部感染合并肺不张患者的临床疗效及其临床价值。方法将137例重症肺部感染合并肺不张患者随机分为观察组及对照组,对照组给予抗感染、吸氧、体位引流、对症支持治疗等常规措施治疗,观察组在常规治疗基础上采用纤维支气管镜肺泡灌洗治疗,对比两组患者的临床疗效。结果患者咳嗽等临床症状消失时间、体温正常时间、白细胞恢复正常时间、炎症吸收时间及住院时间观察组明显短于对照组,比较差异具有统计学意义(P<0.05);观察组肺复张有效率94.37%,对照组有效率78.79%,比较差异具有统计学意义(P<0.05);观察组灌洗后PH值、Sa O2、Pa O2、Pa CO2较灌洗前明显改善,比较差异具有统计学意义(P<0.05)。结论经纤支镜肺泡灌洗治疗重症肺炎并肺不张能够有效解除支气管阻塞,更快地改善症状,提高疗效,减少住院时间。Objective To investigate its clinical value and curative effect of bronchoalveolar lavage through bronchofibroscope in the treatment of patients with severe pulmonary infection complicated with pulmonary atelectasis. Methods 137 patients with severe pulmonary infection complicated with pulmonary atelectasis were randomly and evenly divided into two groups. All patients were given the conventional treatment, which included anti-infection, oxygen uptake, postural drainage and nutritional support. The treatment group was additionally given bronchoalveolar lavage through fiber bronchoscopy. The curative effect was compared between the two groups after the treatment. Results The duration of cough disappeared, body temperature back to normal, leucocyte back to normal, pulmonary inflammation disappeared and hospital stay were obviously shorter in the treatment group than in the control group with statistical significance （P〈0. 05）. The total effective rate was 94. 37% in the treatment group and 78. 79% in the control group （P〈0. 05）. The improvement of PH value, SaO2, PaO2 and PaCO2 was more pronounced in the treatment group than in the control group （ P〈0. 05 ） . Conclusion Bronchoalveolar lavage under bronchofibroscope can improve respiratory tract symptoms and ventilation function, increase cure rate and reduce the duration of hospital stay in the treatment of patients with severe pulmonary infection complicated with pulmonary atelectasis.
