慢性心力衰竭患者预后影响因素的调查分析

摘　　要：目的探讨慢性心力衰竭患者预后影响因素的调查情况。方法分析心内科收治的慢性心力衰竭患者200例临床资料,采用收集资料和查阅文献的方法,对慢性心力衰竭患者的一些影响预后的因素进行探讨和分析。结果两组慢性心力衰竭预后影响因素计量资料中BMI、血液Na+、NT-pro BNP、生活质量评分具有统计学差异(P<0.05)。两组慢性心力衰竭预后影响因素计数资料中的下肢水肿、肝脏肿大、1年内复发具有统计学差异(P<0.05)。BMI增加,患者死亡风险性增加;生活质量评分越低,患者的死亡风险性越高;患者下肢水肿发生率越高死亡风险性越高,差异均有统计学意义(P<0.05)。结论 BMI、生活质量评分和下肢水肿是影响慢性心力衰竭患者预后不良的独立风险因素,通过调查分析均可以为临床治疗和预后观察提供可靠的理论依据。Objective To discuss the influence factors of prognosis for patients with chronic heart failure. Methods Clinical data of 200 patients with chronic heart failure who were admitted to the department of cardiology were collected, and influence factors of prognosis for patients with chronic heart failure were discussed and analyzed via the approaches of collecting data and consulting related journal articles. Results In the measurement data of influence factors of prog- nosis for two groups of patients with chronic heart failure, the differences of BMI, blood Na~, NT-proBNP and scores of life quality were statistically significant （P〈0.05）. In the enumeration data of influence factors of prognosis for two groups of patients with chronic heart failure, the differences of edema of lower extremity, hepatomegaly and relapses within 1 year were statistically significant （P〈0.05）. The higher the BMI, the higher the risk of death. The lower the score of life quality, the higher the risk of death. The higher the incidence of edema of lower extremity, the higher the risk of death（P〈0.05）. All the differences were statistically significant. Conclusion BMI, score of life quality and edema of lower extremity are three independent influencing factors of poor prognosis for patients with chronic heart failure. Appropriate investigation and analysis are capable of providing theoretical evidence for clinical treatment and prognosis observation.
