摘 要:目的探讨盐酸氨溴索辅助阿奇霉素治疗儿童肺炎支原体肺炎的疗效。方法选择本院2012年6月~2014年6月儿科住院的肺炎支原体肺炎80例,根据治疗方法不同随机分为对照组和观察组,每组各40例。对照组予注射用阿奇霉素:10 mg/(kg·d),加入5%葡萄糖250 m L中静脉滴注,每日1次,连用3 d,停4 d后进入下1个疗程;观察组同时加用盐酸氨溴索注射液静脉滴注:〈2岁每次7.5 mg,2次/d;2~6岁每次7.5 mg,2次/d;〉6岁每次15 mg,2次/d。两组疗程均为7 d。比较两组患儿治疗7 d后的疗效及两组患儿的退热时间、咳嗽喘憋消失时间、肺部哮鸣音消失时间,并对两组患儿的肺功能指标用力肺活量(FVC)、第1秒最大呼气量(FEV1)、最高呼气流速(PEF)进行评价。结果治疗1周后,观察组的总有效率为97.5%(39/40),对照组的总有效率为77.5%(31/40),观察组的有效率显著高于对照组(χ2=7.358,P〈0.05)。观察组患儿治疗1周后的退热时间(1.17±0.32)d、咳嗽喘憋消失时间(7.12±1.16)d、肺部哮鸣音消失时间(5.42±0.63)d,均显著短于对照组(P〈0.05)。治疗后两组患儿的肺功能指标FVC、FEV1、PEF均较治疗前显著升高,且观察组显著高于对照组,组间差异存在显著性(P〈0.05)。结论盐酸氨溴索辅助阿奇霉素治疗儿童肺炎支原体肺炎疗效确切,安全性好,可显著改善患儿的临床症状、体征,提高患儿的肺功能各项指标,值得推广和应用。Objective To investigate the therapeutic effect of ambroxol hydrochloride auxiliary azithromycin treatment of mycoplasma pneumoniae pneumonia in children. Methods From June 2012 to June 2014, 80 cases according to the different treatment methods were randomly divided into control group and observation group, 40 cases in each group, the control group was given azithromycin for injection, 10 mg/(kg.d), adding 5% glucose 250 mL intravenous injection, 1 times a day for 3 d, 4 days later, stopped into the next course. The observation group added ambroxol hydrochloride injection vein drop note, 〈2 years old, 7.5 mg each time, 2 times/d; 2 to 6 years old, 7.5 mg each time, 2 times/d; 〉6 years old, 15 mg each time, 2 times/d. Two groups were treated for 7 d. After treatment 7 d, the two groups of children with fever, cough disappeared time of time of wheezing, pulmonary wheezing disappearance time were compared, and the two groups of children with indices of pulmonary function (forced vital capacity (FVC), forced expiratory volume in one second (FEV1), peak expiratory flow rate (PEF) were evaluated. Results After treatment for 1 week, the total effective rate of observation group was 97.5% (39/40), the control group was 77.5% (31/40), the observation group efficiency was higher than the control group (X2=7.358, P〈O.05). The cooling time was (1.17~0.32) d, cough, wheezing disappearance time was(7.12+ 1.16) d, pulmonary wheezing disappearance time was(5.42+0.63) d, respectively, compared with the con- trol group, the observation group was significantly shorter than that in control group (P〈0.05). After treatment, FVC, FEV1, PEF and lung function indexes of two groups of children were significantly increased, and the observation group were significantly higher than those in control group(P〈O.05).
